One of my players recently lost an eye via the Lingering Injuries table in the DMG. I made a custom feat in Homebrew and set it to give her disadvantage on Perception and Ranged attacks. However, only the Perception is working: ranged attack rolls, such as with her darts, are not automatically rolling with Disadvantage. Is this a bug? Is there a workaround?

There is no way to automate disadvantage on attacks. They can roll with disadvantage by right clicking/long tapping the attack.
