If it's public homebrew, you can just add it to your collection and it should be available in your campaigns

You can use the other person's idea as a template to work on making your own similar item, but DDB does not allow players to copy/modify homebrew created by someone else. In large part this is to avoid players 'stealing' someone else's homebrew by making a copy and publishing it as their own.
If you have a subscription you can add the homebrew in question to your collection and use it as if you'd created it, save that you cannot modify it. If you do not have a subscription you'll have to puzzle out how to recreate the item in your own homebrew creations, though I would not publish it at that point.
