I am making a table full of flesh warping afflictions that may be bestowed upon my players when they have been attacked by warped aberrations.
I have a decent sized list already but i'd love to see what you people would be able to come up with.
There are a few limitations: 1. No beneficial effects - I don't want being fleshwarped to be a positive experience in any way 2. Limited negative effects - None of the effects should make it so that a character becomes unplayable
I am mainly looking for cosmetic effects that may have a minor detrimental effect.
Genitals on the face. -4 to any Persuasion checks or Performance or pure charisma checks against someone that can see your face. Does not apply to saves. Please buy a helmet.
Left hand on right arm and right hand on left arm. -2 to sleight of hand checks. Does not apply to saves.
Left foot on right leg, and right foot on left leg. -2 to dex and acrobatics checks. Does not apply to saves.
Internal organs are now external. Anyone rolling a 19 to hit crits you (but does not auto-hit if AC is high enough).
Eyes non-binocular (i.e. like a fish or a cyclops). All ranged attacks are all at -4, even spell ranged attacks.
A third arm sprouts from somewhere on their body and makes 1 unarmed attack against the player per round on the player's imitative, using their stats. You can either have the arm whither and fall off after a certain amount of rounds, or if you wanna go real hard core, it keeps going until it's cut off, which would likely need to be outside of initiative.
Hollow unbreakable arrows are the most OP common magic item, and my current method of coming up with insane combat shenanigans.
if you make a steel pipe with one end closed and a nozzle on the other, you can enlarge it, fill with any liquid, and then drop concentration, creating a high pressure squirt gun. (or a pipe bomb, depending if it holds)
Head has sunken through the shoulders and character's face now protrudes from the abdomen, a loose sphincter like pile of tissue forming within the "neck hole". Configuration might be a problem for a character wearing armor until something is customized.
Buttocks and anus are lifted up to the shoulders, front genitalia being pulled along with the dislocation optional.

Pain. Constant pain. Continuous. Neverending. Pain. You suffer a -1 penalty to all attack rolls and ability checks, and to all Constitution saving throws.
Any way I can think of a tonne of stuff to get inspiration from. Firstly there is all the stuff from the sibriex and its flesh warping table.
I can also think of some nasty diseases which inflict a little body horror to inspire you. Warning the images are very graphic if you look them up.
Tertiary syphilis- Devours flesh, this is the disease people were afraid of when the banished the lepers from society. It should make people avoid you and not want to touch you.
Tetanus- bends spines. This could basically just be a horrifying to watch damage effect as they bend over backwards and scream. Maybe it makes them take damage during rests.
Teratoma- cancer that forms body parts commonly eyes, hair and finger nails in random places on the body. Something nasty like fingernails across the skin could really hurt charisma checks.
Rickets- weak bending bones. Decrease movement speed or vulnerability to bludgeoning
in fungal respiratory infections you can sometimes see fungus growing out of peoples throat
Cleft pallete. Embryos faces actually form in two halves on the side of the head and then fuse together. What if you split them again like an extreme cleft pallete
Some horror monsters and move references. Of course you could use anything from aberrations.
Turning some one minotaur could be pretty horrific with the right description. It's basically a race change.
rutterkin looks pretty wild with its long hand mouth tentacle. Can' equip anything in one hand instead you can only make unarmed attacks with it.
The pale man pans labyrinth. They need to hold their hands out to see and are blinded when both hands are full.

The flesh of the face begins to smooth out, covering nostrils, then eyes, ears, and eventually mouth (and then death because you can't breathe I suppose ... I guess you said to not make unplayable, but this is kind of a GTFO before it progresses too far effect). Everything is still under there it is just all smoothed over into nothing, you could probably cut holes to get at it ...
Hair begins to grow uncontrollably all over the body and continues to grow
All of the player's hair, including eyebrows, eyelashes - everything falls out all at once and doesn't grow back
The amount of skin the player has doubles, and sags out and droops everywhere
The fingers lengthen to 3x their normal length, become spindly, and grow black ragged claw-like nails
The player becomes like putty for a while - so the shape of every blow or glancing touch gets imprinted permanently into their flesh (Ie. a punch to the jaw would leave a twisted face, a slash to the arm would leave a great notch)

Your knees point backwards.
The parts of your brain that let you think before you speak are turned to bone.

A xenomorph bursts out of your chest and starts singing showtunes. Again.
The flesh all over your body becomes covered with a chorus of mouths rendering showtunes, but not in harmony or even the same song.
