I am creating a homebrew magic item that works similar to druid or cleric domains, and grants a number of prepared spells. The user of the item would use their own spell slots, but these spells would not count against their normal prepared spell list. Ie a land druid (of the mountain) always has lightning bolt prepared.
Is it possible to create an item (ie staff of lightning) which then grants a prepared spell like that, and if so, how?

Nope. You could do it as a class feature, but not attached to a magic item nor attached to a feat. The “Add a Spell” subform for Class Features has more fields, including the “Consumes Spell Slot,” and “Always Prepared” dropdowns which can be used.
