Thank you! I don't know how I missed that. However, I don't see a way to add a custom attack that uses a specific equipped weapon, or a way to add static bonuses or dice to a custom attack (i.e. I can't create a custom attack that gets a +1 to hit, or that does 1d10 + 1d8 base damage).
No, a custom attack can’t use a weapon. That’s what the weapon’s regular attack is for.
No, it can’t combine dice like that. And you can’t add the +1 to a custom attack, but you can customize the weapon and add the +1 to that. (Weird, I know. 🤷♂️)
What exactly is it your tryin’a do? You’re trying to make the 1d4 bonus action PAM attack using a +1 weapon? What does that have to do with either a d10 + d8?
My DM made a custom glaive (1d10) that deals 1d8 bonus lightning damage. However, the Polearm Master feat displays the opportunity attack with the wrong reach and without any damage; it also doesn't add the 1d8 to the bonus attack damage for Polearm Master. So I figured I'd try to add custom attacks with the right numbers. I'm guessing that the wrong information is because it doesn't see the polearm (even though on the character sheet, it's marked as wielded), but I'm not sure if there's a way to fix that either.
The Reaction is more of a placeholder to show that you have a special opportunity attack, but you just make a regular weapon attack with the polearm weapon. It's only the Bonus Action that needs special customization/custom actions to account for the 1d4 die.
It would still be helpful for the Polearm Master bonus action attack to be calculated properly (the reach, the attack bonus, and the damage) and to use the polearm that my character is wielding. I don't have any way to create a custom attack that deals 1d4 + 1d8 + 6 damage (1d8+1 of which is from the magic of the weapon), and it's a nuisance to have to manually type out the formula every time I hit with the bonus attack.

Is there a way to add a customized attack to my character sheet?
Alternatively, is there a way to show the bonus attack for the Polearm Master feat, using the magic weapon that my character is wielding?
Yes, you can add custom attacks directly to a character sheet:
