The way WotC designed magic items to work this edition is to be static after creation and therefore explicitly not scale with/for their bearers like that. That being the case it is not possible to program them like that in DDB’s system.
I am trying to create an Item that scales with its Owner.
I want the wearer of this ring to have a +X Bonus to a Skill of their choice, where X is 1 for every four levels they are...
Has anyone done anything like this?
The closest you could manage is an item that gives expertise in the skill and a -2 penalty to the skill since your proficiency bonus = 2+1 every 4 levels

It’s actually more like 2+(1 every 4 levels starting at 5th level, max 4), so it caps at a +6 maximum. So it’s gonna hit at the same levels as PB, and cap at a total +4 at 17th level. On top of that, if they already have either Expertise or Twice Proficiency then it wouldn’t stack.
They could perhaps give a bonus equal to the proficiency bonus in another way though:
