I rolled a 7 with +4, double dice is 14+4=18, if sneak attack applies yeah I guess it doubles to 40 so: 18+40+6=64 total damage?
Ok. Re-reading I'm seeing what you said. Do I double what I rolled as above or roll again and add to it? If so rapier extra damage 5 and sneak extra 14
So(assuming sneak applies) I did 64 damage or 11+6+20(initial rolls) +3+12(extra rolls)=52 damage.

I'm not sure, hell. I only rolled damage on the dagger, not a 2nd attack(here 10) so my totals may be way off. Bossman will clear it up for us. Thanks/ sorry DM

Ctenmir pulls away from Cleese and wipes the blood off his mouth with his shirt sleeve. Any half decent vampire would have used a silk napkin. It is becoming clear why he contents to haunt the back of a tiny crypt doing goon work for another villain. The poor soul wouldn't last a week in Barovia.
Ctenmir HP: 96
Karnys blasts Ctenmir with lightning (DC 13, 33 damage), then moves in to provide a distraction for Roderick.
Ctenmir Dex Save: 10
Roderick shanks the vampire for 46 damage. Ctenmir lets out a wolf-like howl as his noble blood stains the ground. (I myself confused by the rolling, but I can say that the second attack did not hit and Roderick did get his sneak attack damage.)
Cleese: If Ctenmir attacks you or one of your allies you will have advantage when you repeat the save.
Ctenmir's coat billows and his skin begins to roil. Where he was once standing, there is now a cloud of green mist that darts past you and out the door.
Cleese: Your Religious knowledge informs you that vampires have a powerful regenerative ability. Ctenmir could return to full health in 24 seconds flat.

Unsure on the layout of the field sneak attack 17 may apply. Do I have a non-incapacitated ally by him?
