A LV 13 party (paladin, rogue, warlock and bloodhunter) are fighting a giant cosmic worm, Astrum Deus. Each of Astrum Deus' segments has about 12 health, and it has 42 segments total.

The players brought the wrong characters and end up playing as 2nd-level characters in an 8th-level campaign.
A wizard that loves mirror image and phantasmal force, a melee-focused fighter, a barb-druid, and an artificer that the dm somehow allowed to invent guns that fool.
the npc also had a cheating magic weapon that applies damage over time and makes healing nearly impossible.

8/11 pretty bad but fiend-locks are allowed non-evil alignments so that does not mean they immediately will try to kill each other if we apply the same thing here. The paladin will probably fail to kill the old one anyway considering, you know, great old one

11. The weak swordsman clings to his instrument. It is better you have a sword, but death must lie under your fingernails, if need be. Learn death with your elbows, death with your knees, and death with your thumbs and fingertips. It is said death with the tongue is useful, but I find words too soft an instrument to smash a man’s skull.
2/11. Depends what the party is made of, but if they have ranged attackers, wood elves, magic, poison resistance, etc. and/or stay out of range of the sting, they should be fine.
Party of two wizards, one barbarian, and one sorcerer. Sorcerer runs out of spell slots and somehow has no damaging cantrips, wizards are cornered by minotaur skeletons, and archvillain (transmuter/necromancer) True Polymorphs the barbarian into a frog and puts him in a jar.
8/11 sorcerer needs to hit the barb to free them. The fact that you are fighting an opponent with true polymorph means they are high level, so the minotaur skeletons should be fairly easy if the fight was meant to be winnable at all unless there is a large horde of them. But to be honest, I am surprised a sorcerer with 0 damaging cantrips made it that far unless there is something I am not being told, so being down 2 members unless the barb gets hit is really bad.

Depends how "high-level" the party really is, but probably 9 to 11.
