My first player to die as me DMing was my cousin's Gold Dragonborn Gunslinger in Tomb of Annihilation that was trying to become a vampire. It had become a vampire spawn, and was breaking the game, and I was a new DM and didn't know how to handle this, so I had them have to do a quest to get a bucket of lava from a volcano, and they fell in, as the cave entrance was windy, and 2 of them died.

I haven't gotten any of my players killed, but 2 of them got sold of to slavery after a very funny critical failure. After some tense cobbled together fighting where one of my players almost got his head cut off by a nobles personal guard (i Shouldn't have put half-dragon veterans in front of level 2 characters, and there are no clerics in the group.) But the city watch intervened and sorted everything out in a even harder to cobble together role play scene.
In my games, chickens aren't native to Earth or Toril, they're invasive, and all shapeshifters that can turn gargantuan once a day, and frighten people, and other such abilities. This came about from a funny story in my ToA campaign with Volo, rolling a natural one on what a Chicken is, so he referred to them as the "Legendary Chickens of the North", and told of their terrifying powers, frightened of them like dragons. In a separate campaign almost as a joke, I had a boss battle against a baby chicken CR 7, at about level 5.
