You can tell because: it says "Version" instead of "Source".
It is showing 1d12 damage, when the real Toll the Dead would be showing 2d8. The cantrip shows the initial damage (d8s) and the higher conditional damage (d12s) in the Notes field. Instead the homebrew version you're using is listing 1d12 - so it's not even set to scale properly.
Did you add this to your collection or are you in a campaign? If added to your collection you should remove it. If in a campaign, try removing your character from the campaign to see if that helps.
Go into the Character Building (Edit), go to the Home page and turn off Homebrew (it will go grey) then go to Class, then Spells, open Known Spells section, and next to the Toll the Dead cantrip, click the "X Remove".
If the link changes to Learn it has been removed. If it doesn't, can you advise what happens when you click it?

It appears that someone made an attempt to create a home red version of Toll the Dead that actually puts the d12s as rollable on the Character sheet since the regular version does not. It seems like that spell has possibly caused your character to fall victim to the same bug as this other user:
Tip for future, instead of a homebrewed spell to do it, it is fairly easy to simply add a custom action directly to the character sheet for that. An even easier solution is to use the feature built into the dice roller that allows players to make a custom roll, simply hit the little red icon on the bottom left corner and you can select an appropriate number of any dice size one wishes, in this case between 1 and 4 d12s depending on character level.
That is all only temporary anyway since DDB is already actively working on a series of system upgrades that, among other things, would make the d12 option for Toll the Dead as clickable for us.

I tried removing myself from the campaign and removing the spell, and i still got the same error. It just gives an API error.
