Here are the things I noticed in the last half of the PHB.
As a general point, I wasn't sure whether or not it made sense to link self references to the various actions, conditions, and skills within their own definitions so I left those out.
Chapter 6: Customization Options
In the Great Weapon Master feat, the word heavy in the second bullet point should link to the heavy weapon property.
Chapter 7: Using Ability Scores
In the Proficiency Bonus section, the word History in the last sentence of the 4th paragraph should link to the History skill.
In the Skills section, there are a number of reference to the various skills that should be linked.
In the Passive Checks section, the word Dexterity in the last paragraph of this section should link to the Dexterity section in the chapter. Also, the Perception in the third paragraph should link to the Perception skill.
In the Working Together section, the word Help in the first paragraph should link to the Help action.
In the Group Checks section, the word Survival in the third paragraph should link to the Survival skill.
In the Strength Checks section, the word Athletics in second sentence should link to the Athletics skill.
In the Dexterity Checks section, the words Acrobatics, Sleight of Hand, and Stealth in second sentence should link to their respective skill.
In the Hiding text bubble, words Stealth and Perception (when not used as a paragraph header) should link to their respective skills.
In the Intelligence Checks section, the words Arcana, History, Investigation, Nature, and Religion in second sentence should link to their respective skill.
In the Wisdom Checks section, the words Animal Handling, Insight, Medicine, Perception, and Survival in second sentence should link to their respective skill.
In the Finding a Hidden Object text bubble, word Perception in the first sentence of the 1st paragraph and the third sentence of the 2nd paragraph should link to the Perception skill.
In the Charisma Checks section, the words Deception, Intimidation, Performance, and Persuasion in second sentence should link to their respective skill.
Chapter 8: Adventuring
In the Travel Pace section, the word exhaustion at the end of the the second paragraph should link to the exhaustion condition.
In the Jumping section, the word Athletics in the third paragraph should link to the Athletics skill.
In the Other Activities section, the word Perception in the first paragraph should link to the Perception skill.
Chapter 9: Combat
In the Your Turn section, the link to the Movement and Position section should be in quotations. Also, the words Dodge and Ready in the fourth paragraph should link to the Dodge and Ready actions respectively.
In the Being Prone section, the terms drop prone in the first sentence and Standingup in the second sentence should be bolded.
In the Modifiers to the Roll section, the words finesse and thrown in the second paragraph should link to the finesse and thrown weapon properties.
In the Opportunity Attacks section, the word Disengage in the third paragraph should link to the Disengage action.
In the Two-Weapon Fighting section, the word Attack in the first sentence should link to the Attack action. Also, in the first paragraph the word light should link to the light weapon property and in the second paragraph the word thrown should link to the thrown weapon property.
In the Grappling section, the word Attack in the first paragraph should link to the Attack action.
In the Shoving a Creature section, the word Attack in the first paragraph should link to the Attack action.
In the Mounting and Dismounting section, the word prone in the second and third paragraphs (4 times total) should link to the prone condition.
Chapter 10: Spellcasting
In the Concentration section, the word incapacitated in the last bullet point should link to the incapacitated condition.
In the Attack Rolls section, the word incapacitated in the second paragraph should link to the incapacitated condition.
Chapter 11: Spells
In the description of the Anitmagic Field spell, the words magic missile in the Targeted Effects paragraph should link to the magic missile spell.
In the description of the Haste spell, the words Attack, Dash, Disengage, Hide, and Use an Object in the last sentence of the first paragraph should link to their respective actions.
Appendix A: Conditions
In the Grappled section, the word incapacitated in the second bullet point should link to the incapacitated condition.
In the Paralyzed section, the word incapacitated in the first bullet point should link to the incapacitated condition.
In the Petrified section, the word incapacitated in the second bullet point should link to the incapacitated condition.
In the Stunned section, the word incapacitated in the first bullet point should link to the incapacitated condition.

Thanks for the quick update! You got almost all of them. Here are the few I noticed that managed to slip through.

And in case anyone is interested, the general process I've been using to identify most of these issues is to go to each page and do a regex search (you'll need an addon/extension for your browser for this) using the following regular expressions:
Actions: "\b(Attack|Cast a Spell|Dash|Disengage|Dodge|Help|Hide|Ready|Search|Use an Object)\b"
Skills: "\b(Acrobatics|Animal Handling|Arcana|Athletics|Deception|History|Insight|Intimidation|Investigation|Medicine|Nature|Perception|Performance|Persuasion|Religion|Sleight of Hand|Stealth|Survival)\b"
For the conditions and weapon properties searches it's important to make the search case insensitive to catch those instances when they appear at the start of a sentence. The search for weapon properties will often turn up a lot of false positives since the words range, reach, and special are used so commonly in other context.

One of my players has a Wisdom of 15, giving him a +2 to spells, yet when we look at his Electronic DDB Character sheet the spell (for example) Cure Wounds only shows 1d8+2, and does not factor in the Disciple of Life bonus.
Is this a mistake of the DDB Character sheet or was this done on purpose?

Ooh, it doesn't either. Also, it should be a +3 because the ability is 2 + the spell's level, so Cure Wounds cast as a 1st-level spell would increase the healing done by 3.

This is a modifier that we have not been able to implement yet, but it's on the list.
Making a Divine Soul in the character builder and in the text it says you can unlearn the spell given by your Divine Magic choice, as long as it is replaced with another cleric spell, but I can't seem to find a way to unlearn the Divine Magic Spell.
For example at level 1, I choose 'Chaos' and get the 'Bane' spell. i get to level 3 and want to change 'bane' to 'spiritual weapon.' This should be possible.
I see earlier there was an issue where it wasn't giving the spell. Now I have the spell but can't seem to retrain it.
Is this a bug or am I just not finding the right spot to unlearn this spell?

I don't know if this is a bug or if it is meant to be this way, and I haven't seen it here, so I figured I would share it just to be on the safe side. The back drops that came from preordering the book included the cover art of Xanathar looking over Sylgar. When this one is chosen the backdrop is just the very right side of Xanathar, on the left side of the screen. There is essentially no image; just a white backdrop with a quarter of a beholder.

I noticed that with Gloom Stalker, the extra spells are not automatically being added to my spells known.
Starting at 3rd level, you learn an additional spell when you reach certain levels in this class, as shown in the Gloom Stalker Spells table. The spell counts as a ranger spell for you, but it doesn’t count against the number of ranger spells you know.
This is partially fixed. If you're a single-class ranger, it will work fine now. If you multiclass, it won't calculate levels just right. Pretty late in our part of the world, so we're going to push a fix for that in the morning.

This doesn't appear to be fixed yet - my monk 7 / ranger 3 is showing the ranger spells granted at 5th and 9th level. Gnarf
Also noted that darkvision is not stacking correctly still - Gnarf should have 150ft, however the senses tab is showing 180. It is adding the full 60ft from Gloom Stalker rather than just the 30ft.

Under Crafting Magic Items, mariner's armor isn't tooltipped.
