I've been looking for a way to modify these from the character sheet but can't seem to do it (mostly as a way to implement some madness modifications).
I've been loving that most things can be changed just surprised these can't. Even tried making an item that have -1 which didn't seem tonwoto either.
currently there's no way to customise the initiative of a character directlyt via the character sheet - that may be coming with future updates to the character sheet though.
What you could do is create a homebrew "madness" feat for each player and then define the properties of it - that would allow you to apply a bonus/penalty to initiative.
With regards proficiency, that's such a core part of the game progression that there isn't a way to customise it within D&D Beyond.
Adjust proficiency in what way? For abilities and saving throws you can simply click the dots. The manage icon at the bottom of the box opens the sidebar for further changes like adding new skills. For things like proficiency with armor and stuff these are determined by class and other sources and cannot be changed without changing those sources.
However, you can create a feat or item with: Add Modifier >> Type: Bonus >> Sub-type: Proficiency Bonus >> Fixed Value: ??

Your proficiency bonus is based only on your character level, so multiclassing doesn't affect it! Much like increase in damage for most damage-dealing cantrips.
Your proficiency bonus is always based on your total character level, not your level in a particular class. For example, if you are a fighter 3/rogue 2, you have the proficiency bonus of a 5th-level character, which is +3.
Additionally, the D&D Beyond character sheet will handle all of this for you!

For whatever reason this will not work for me. I'm adding the bonus like you described (see blow), but the character with the feat does not get an init bonus. Am I doing something wrong or did this just stop working?
After you add anything to, change anything on, or remove anything from any homebrew, you MUST re-save the entire thing using the main [SAVE CHANGES] button under Basic Information or the system won’t pick up your changes. After that it can sometimes take up to 20 minutes for those changes to push through to the character builder/sheet.
Thanks for the advice. I added a Guardian Feat for the Magic Item Property. But the list of Homebrew feats is currently 3,025 pages long. I managed to find all the many Guardian feats around page 1345 by fiddling the URL, but we need search by name & creator, at least! And I cannot find my own Guardian feat in the list, so I cannot tweak it.

We have a homebrew rule that makes your initiative based on either dex or wisdom. How would you set that up as a homebrew?

Hey just checking in. Is there still no way to adjust proficiency on a character sheet?
Do you mean the Proficiency Bonus, the Skills, or individual Proficiencies like weapons, armor, languages, and tools?
