It lashes out with its tentacles against Sir Onyx and Sir Glint.

But without taking an action to investigate it with a medicine check, you're not able to determine whether that indicates significant injury or just a quirk of the monster)

Sir Onyx's blade carves pieces out of the monster, lopping off two tentacles. Each strike causing the voices to scream in inhuman fear.
Sir Glint's thrust strikes true as well, holy energy burns into the thing through his blade, burning the flesh around the wound.
Sir Omen's arrow flies over Sir Glint's head to bury itself the beast's hide.
