(@Wile_E_Coyote, the first round doesn't contain any combat actions, do you consider it as an out of combat round? Gravity's maximum HP will be reduced to 36, but she has temporary hitpoints.)
Technically Phenex uses a bonus action every round to maintain a spell that takes away hp. I'd call that a combat action.

The incubus unhooks the chandelier, causing it to crash to the floor. It narrowly misses several chanting tieflings, but does not interrupt their recitation. The room is now dimly lit by wall sconces and lit by the wall of radiant fire (how far, if at all does that extend Tesservett?)
The incubus flues to the northwest corner of the room and once again attempts to hide.
Sam and Ana can make checks to see him from where they are, I think anyone else would have to reposition. Remember I'd relying on darvision and you're in dim light range it's with disadvantage.
Tetrevess hears a voice in her mind, it's smooth and seductive (it's canonical gender fluid, but Phebex was most recently and currently in the male incubus form, I'm not sure that this would come across in telepathy. Regardless, Tetrevess would infer that it is Phenex). The words it conveys are suggestive of an idea that might have been the wizard’s own. "Come now, wizard. You know you've a mire useful spell up your sleeve."

Gravity will toss her Feather Token(Bird) upwards so that it floats over the head of the half-dragon briefly, then the token disappears and an enormous, multicolored Axe Beak (roc) takes it place. The bird will occupy the space of the Paladin while trying to squish him under its weight, thus blocking the entrance to the alcove.
Ana steps away from the giant apes, more for space than anything else, then turns her attention once more to the demon. She raised her pact weapon and shot two arrows at it.

(@Wile_E_Coyote, Thanks! She did throw the feather upwards (5 ft), but the bird will occupy approximately 20 ft space in each dimension or more, so physically its leaving no space for fall damage. But it's possible for Pentival to move away from the occupied place before the transformation process. I hope that the explanation is enough for you to resolve the issue.)

The gargantuan bird fills a huge area if rhe room, its feathers catching the edge of the radiant wall, the bird begins to freak out. The half dragon, finding itself between the legs of the enormous bird grabs on to the bird's tail and attempts to climb atop it (see, 2 size categories. If the girallons weren't dead, they'd be way better at this).

You're the DM so you could decide otherwise but in general Hiding in plain sight is not allowed by the rules. From PHB: "You can't hide from a creature that can see you clearly, and you give away your position if you make noise, such as shouting a warning or knocking over a vase. An invisible creature can always try to hide. Signs of its passage might still be noticed, and it does have to stay quiet." Ana has Darkvision so she can see in dim light as if it were bright light and there's nothing there for him to hide behind.
