The Fang of Faerun Comes from the forgotten realms and there are portals to the Elid'nas and other “local” planes but as far as I know no permanent portals to other worlds.
Perhaps there is something in the wilderness or Leoni Territory?

All right... if you've ever seen The Nightmare Before Christmas, there is a circle of trees with doorways to the different 'personifications' of Holidays.
Far away from any settlement Human or otherwise, and it does some weird stuff to spirits, the Planar Circle resides. An ovaloid shape of portals is obscured in fog, and a powerful entity from each plane will guard its portal with its life, though it can be reasoned with. Once the entity has appeared, the portal will open for 10 minutes.

Southwest of the land ruled by the Sirens is a town called Kolt, the people of Kolt rejected visitors from the other realms and blamed them for the calamity. This leads then into conflict with a lot of the groups surrounding them despite that they maintain their independence through faith and fire arms. Gun Smithing took of in Kolt before the calamity and many of the supplies needed for it are found around the town

A cult called the Ironbloods yet persists in Kolt. Originally a guild of metalworkers and gunsmiths, they survived the Calamity by hiding in a cave rich with iron deposits. The cult remained underground for many years, their practices becoming twisted and strange. Their current practices include taking a strain of gunpowder as a drug, purportedly to be able to see more creative ways to build contraptions and firearms.
Today, entering the cave could mean a swift death, as the Ironbloods have had many, many years to fortify themselves against intruders with traps. Their leader, Gilios Davar (a high elf) is extremely paranoid and is the individual responsible for ordering such fortifications to be made.

The outposts have different reactions to other worldly visitors ranging from normal racism to shoot on site. This has led to the creation of a magic metal in the area known as cold iron. It hurts creatures from other planes when they touch it. Luckly It's not very common and tends to be used more of status symbol then as a weapon.

Initially, it was home to many decomposing agents, but those, weathering, and the passage of time fossilized the splintered trunk similarly to the stump. This became the Ilissar hill range, which reaches into Leonin territory.

Did the office job thing once to a tarrasque. The tarrasque got meager pay and no insurance. Needless to say, when the car crash happened, that tarrasque wasn't getting another car. (his pay wasn't good enough. The tarrasque didn't die.)

It's been ages since I've been on this thread. I do believe that it's supposed to be about creating a specific world, as you can read. However, if you want to add an element from one of your worlds, I'm sure nobody's about to object.
Sorry. the world John is from is Xyltherx, where the magic and the mundane combine in harmony. Elves live in giant apartment complexes in the wilds, and beasts get cars and other things. It's a good way to be able to smoothly introduce a goblin mafia, or an illegal animal smuggling ring. If you want to get away from the boring, then go to the Sea of Salt. It has a couple islands inhabited by the Cryl, crystalline beings that have to mine for water crystals and then put them through the refinery to get pure water. The sea is made of pure salt, and all the sea monsters who live there are a bit crystalline. They don't need water, either.
the pasta gods created the world. FSM, Ramenu, Satay, Yahweh, and Princess Kenny. The world is now like modern day, with a radicalist orc as trump and tieflings living predominantly in australia.

A dark lord of ancient legend is depicted on ancient ruins slaying gods and other powerful ancient beings with a mighty Spear. Other runes depict the Spear absorbing the powers of the gods into itself. Finally in a lone abandoned temple deep within the most dangerous region of the world is the tale of the dark lord's banishment and the loss of his Spear leaving it hiding somewhere on the material plane still imbued with the power of 1,000 dead gods.
