In my previous session a Karen named Ka Ren (a joke villain) turned into a nightwalker because of crazy RNG and reality warping stuff.
She's my new BBEG and is gonna revive an army of undead in order to create a distraction to try to stop the players from dealing with her, so that she can berate the ultimate manager, the king. I am linking what was planned to be my BBEG to Ka Ren by having him and his followers worship Ka Ren as a emissary of their god.
I have a joke BBEG as well it is BILBO OF THE SHIRE. This was a hafling from a place called the shire named bilbo, and he commanded and army of his own clones. He also commanded an army of sams and frodo's and destroyed a cloud giant kingdom with sheer stupid ness. But my favorite is the hag queen of my home brew setting hags finger. She commands the hags of this island with an iron fist and strives to conquer the city of barg the only settlement on hags finger. This is what she looks like

The favorite I've run so far was Kauliste, an ancient grootslang posing as an avatar of the Yuan-ti deity Merrshaulk. Kauliste was quickly gathering an army of Yuan-ti with the ultimate goal of conquering the known world and becoming truly deified by the Yuan-ti's worship and rituals.
A lot of why Kauliste was so fun to run was the political intrigue. The players were able to cause a schism within the Yuan-ti infrastructure with doubt of Kauliste's divinity, and eventually insight a rebellion of "heretics" against her, as opposed to simply attacking her head-on.

One of my favourites was King Squeek, a half-fiend wererat necromancer who had armies of rats, wererats, undead and combinations of all three, and who could more or less spy on anything the PCs were planning and counter it, either with intrigues in town or ambushes...

Ooh, this is hard. I've done both Dungeon of the Mad Mage and an extended version of Dead in Thay, and they both have some good villains (Halaster Blackcloak and Szass Tam in particular), but I would have to say that my favorite homebrew BBEG was Theren Liadon, Scion of Orcus (partially based off of Matt Colville's Kaleral the Vile).
Theren Liadon, a necromancer had a dark-grey, hexagonal tower made out of dead bodies that he had collected from across the realms. He needed to get his tower to a certain height (6 stories, IIRC), and then he would be able to summon Orcus.

Well, I'd have to say that since we're talking about ones we've run, I'll go with Acererak the Demi-Lich from S1: The Tomb of Horrors (no, gentle readers, I didn't put any home PCs through this meat-grinder of a module, only Convention goers).
If I have to pick an overall BBEG from D&D, I'll go with Iuz the Old.
If we're talking about BBEGs from anywhere, then Sauron is my all-time favorite, even if he's really only the BBEG's lieutenant....

Iuz is a demigod. I think the child of Graz'zt and Iggwilv(Or Tasha, but I'd rather not get into an argument with Brooklyn_Red_Leg about it, since I don't know very much about Greyhawk.)
My favorite BBEG would probably be either the Earl of Squirrel that I'm using in my current homebrew campaign, or Immeral/Kruzar Necrosyse, who are a necromancer I played as and his enchanted camel. Immeral was a soulless neutral-evil high elf necromancer who found the Book of Vile Darkness. Kruzar is his undead camel companion/steed, who has a multitude of enchantments that have been cast on him, and has the same intelligence and spellcasting abilities as Immeral. They both became liches, and invaded the entire planet with their undead army, starting with the Underdark. After that, they decided to go to other planes of existence, and to retrieve Immeral's soul from Fraz-Urb'Luu.
My favorite so far is probably Calius, the Dark Angel, one of the main villains on the Adventurer's Tavern thread. He's a fallen angel who believes that the angels of the Upper Planes just use the so-called "common good" to excuse the atrocities they've committed. He's lived for thousands of years, and though he himself is relatively powerful, it's his schemes that make him dangerous. Not once, but twice, he's made the heroes' attempts to kill him work out in his advantage. Now, due to one of the heroes, he was raised from undeath (long story) and practically became a demigod. This was when the hero in question was on a quest to kill him.
He has layers and layers of plots, stretching back for millennia. What seems to others like a straightforward move is actually just a tiny action in a larger game he's been playing since the start of time. Oh, and with midnight-black wings, steel-gray plate armor, and ghostly white eyes glowing with radiance, he looks awesome.

Hmm... yeah, that sounds like it'd be a running joke in that campaign. Calius is more of a serious villain who poses a significant threat to all of Lesta and beyond. Oh, and most people in the tavern hate him more than Eris, and that's all you really to know.
