Choose one creature or nonmagical object that you can see within range. You transform the creature into a different creature, the creature into a nonmagical object, or the object into a creature (the object must be neither worn nor carried by another creature). The spell lasts for the duration, or until the target drops to 0 hit points or dies. If you concentrate on this spell for the full duration, the spell lasts until it is dispelled.
This spell has no effect on a shapechanger or a creature with 0 hit points. An unwilling creature can make a Wisdom saving throw, and if it succeeds, it isn’t affected by this spell.
Creature into Creature. If you turn a creature into another kind of creature, the new form can be any kind you choose whose challenge rating is equal to or less than the target’s (or its level, if the target doesn’t have a challenge rating). The target’s game statistics, including mental ability scores, are replaced by the statistics of the new form. It retains its alignment and personality.
The target assumes the hit points of its new form, and when it reverts to its normal form, the creature returns to the number of hit points it had before it transformed. If it reverts as a result of dropping to 0 hit points, any excess damage carries over to its normal form. As long as the excess damage doesn’t reduce the creature’s normal form to 0 hit points, it isn’t knocked unconscious.
The creature is limited in the actions it can perform by the nature of its new form, and it can’t speak, cast spells, or take any other action that requires hands or speech, unless its new form is capable of such actions.
The target’s gear melds into the new form. The creature can’t activate, use, wield, or otherwise benefit from any of its equipment.
Object into Creature. You can turn an object into any kind of creature, as long as the creature’s size is no larger than the object’s size and the creature’s challenge rating is 9 or lower. The creature is friendly to you and your companions. It acts on each of your turns. You decide what action it takes and how it moves. The GM has the creature’s statistics and resolves all of its actions and movement.
If the spell becomes permanent, you no longer control the creature. It might remain friendly to you, depending on how you have treated it.
Creature into Object. If you turn a creature into an object, it transforms along with whatever it is wearing and carrying into that form, as long as the object’s size is no larger than the creature’s size. The creature’s statistics become those of the object, and the creature has no memory of time spent in this form, after the spell ends and it returns to its normal form.

The spell states that you can not use any of your equipment while transformed, referring to the equipment you would have been wearing while transmuting, so it should be possible to simply lay it on the ground before casting the spell and then just pick it or anything else up afterwards correct?
I'm sorry, I was reading comments to see what the community thought of various uses of this spell, when I saw yours. It is causing me intense stress. It is "for all intents and purposes."
The DM could choose to modify or home-brew an item or spell to cause a permanent True Polymorph
It is for all of the transformations.
This combined with simulacrum is just ridiculously overpowered. Create a simulacrum of you at level 20, then turn it into a nightwalker or pit fiend.
The treasure chest might already be a mimic. Fun twist: as you attempt to transform the treasure chest into a mimic, something goes wrong and nothing happens. Upon closer inspection (aka poking), the treasure chest is revealed to actually be a mimic.
So I'd like to play Simic Hybrid Warlock, use this spell to permanently transform into Ancient brass dragon and use the change shape ability to turn back into myself. What happens to my Animal Enhancement and can I still cast my spells and use anything I could have used before like proficiency, feats,
background feature and so on?
I think it’s up to your DM, ultimately. RAW, it says your statistics would be replaced by the creature’s. Then, a dragon’s change shape action says they transform into a beast or humanoid and, as far as I know, Simic Hybrid’s are humanoid so that works fine. The most important part of a dragon’s change shape is the last sentence: “Its statistics and capabilities are otherwise replaced by those of the new form, except any class features or legendary actions of that form.“ So, it is up to your DM to decide what this spell means when it refers to your “statistics” and whether that includes your class features.
To the side of the argument that would support having your class features, this spell states: “The creature is limited in the actions it can perform by the nature of its new form, and it can't speak, cast spells, or take any other action that requires hands or speech, unless its new form is capable of such actions.” This to me says that you would retain your ability to cast spells and any other ability from your class because dragons are known to be able to become spellcasters.
Tldr; If I was your DM, I would absolutely allow it.
Get this, chain simulacrum creation via wish spells and then you have infinite pit fiends. Ludicrously overpowered. Have fun!
thanks for the help
yes
you're wrong, it specifically says "The spell lasts for the duration, or until the target drops to 0 hit points or dies."
So technically speaking if you wanted to you could true polymorph a tarrasque into Tiamat if you really wanted to
Could this theoretically turn one party member into a clone of another for a time?
What happen if you cast the spell on a non willing creature? Say I turn the BBEG into a snail or a toad, will this work? And if I kill them in this duration, do they stay dead?
If they fail the save, it will work, but if you kill them in their new form they will return to normal.
However, you could turn them into a Brick or something, then maintain concentration for the full duration so they stay that way until the Spell is dispelled, and just bury them in the dirt or something.
With a wizard, can I True Polymorph myself into say; a Pit Fiend, and then afterwards Shapechange into the Pit Fiend and then mix and match traits?
If you choose a dragon that has change shape and assume your own form? Are you still a dragon?
It is tho :)
YES
