In "16. Magic Cauldron", the "Nylas Jowd" entry at the start of the section should be part of the block-quote that follows it. Also, in the first sentence of the "Illusory Wall" entry in that same block-quote "area 18" should probably link to "18. Illusory Walls".
Your entry for Otto, the Faerie Dragon has an omission (the same omission found in Faerie Dragon (Older), by the way). The Innate Spellcasting trait for Otto does not contain the spells he gained when he was a younger dragon, despite "mage hand" being directly mentioned in Otto's tactics in Dungeon of the Mad Mage:
Level 8, Area 2: This is probably going to be another case where the original text is wrong, but since I don't have a copy of the physical book, here goes: In the area description it says that there are three bullywugs present.
Bullywugs. Three bullywugs use their Swamp Camouflage feature to hide in the swampy tunnel leading to area 19.
But later in the area description, it says:
When they spot the characters, three of them try to sneak away to areas 19a and 20, to warn the rest of the tribe. The others wait to see where the intruders go.
If there are a total of three bullywugs, and three try to sneak away, then there would be none remaining to "wait and see where the intruders go".
Level 8, Area 19c: Again, this may be an issue in the book but I don't have the book, so here you go. Part of the area description reads:
Platform. Between the two middle statues, a mostly submerged ramp rises to the top of a dry platform, at the back of which is a closed door that opens into area 19c.
Hyperlink leads to the wrong location. Level 6, Room 47a, south gate arch. Hyperlink says, "area 6 on level 18" (which is correct) but leads to area 6 on level 16. Thanks!
Another creature outside the orb can use an action to attempt the check if its within reach of the restrained creature; on a successful check, the creature is freed from the orb.

Also, as an aside, I appreciate that D&D Beyond does things like this, as it makes it easier to run the game. Small details like the addition of hit dice and a challenge rating for the animated staff are nice touches which the book left out.

Looking through Level 14: Arcturiadoom I came across a few minor issues that could probably use correcting.
In "13b. Alteration Chamber", in the "Mithral Key" entry in the last sentence "area 37" should probably link to "37. Weapon of Mass Disintegration".
In "14. More Scrap Iron", in the first sentence "level 13" should probably link to "Level 13: Trobriand’s Graveyard", and the last sentence "area 12" should probably link to "12. Melairkyn Foundry".
In "15. Mecha-Halaster", in the last sentence of the first paragraph "18" should probably link to "18. Death’s Head Guard Post".
In "Arch Gate to Level 9", in the first sentence "Gates" should probably link to the "Gates" section in "Undermountain Overview".
In "33. Doomcrown’s Quarters", the "Monsters" entry should be part of the block-quote the follows it.
In "Using the Mithral Keys", in the second sentence there should be a space between "V" and "with" in "Vwith".

In the "Meeting the Headmaster" sidebar, in the first paragraph there should be a paragraph break between the 4th and 5th sentences.
In "4. Fresh Water Fountain", the "Fountain" entry at the start of the section should be part of the block-quote that follows it.
In section "15a. Headmaster’s Office", in the first sentence of the "Pneumatic Tubes" entry "level 10" should probably link to level 10 or to area 4b of level 10.

Level 22, area 40: minor typo
It should read, "loses the pillars’ protection if it leaves the room" (or maybe when it leaves the room). It's just missing the word "if" or "when".
Level 23, area "Halaster's Tower": incorrect area number reference
These area numbers are incorrect. The correct numbers are "areas 19 and 22".
