I have been trying to create a homebrewed prototype weapon. My DM has approved it, but wants final word. However, there is no way that I can find, to create an sentient item with home brew.
It is literally as easy as finding a weapon you like and adding some flavor text about its personality. Most of the time, giving sentience to a weapon influences how it reacts to the person wielding it and in some cases can change how the wielder acts. For example, a sword that is evil in nature may try to influence the wielder to do evil things, such as kill the first creature near them if they have not had the taste of blood in a while. Something like that would require a Wisdom save or something like that of course. On the opposite end, an axe may have a passivist nature and you may have to convince it to attack in the middle of battle. That might require a persuasion check or a Strength check to overpower its resistance. Sentient weapons could just be a companion as well. Maybe an ancient elf fused their soul into a spear to extend their life and help adventurers, making the spear a +1 or +2. You could also make the weapon a friend who has passed away and now keeps the wielder company. You can take any weapon and any magical ability and throw sentience on it. If you are starting at a low level though, I would suggest nothing crazy on the magical abilities. Keep it simple at first.
To put it more simply since neither of the other posters mention it - to create a Sentient Magical Item, keywords "Magical Item", create a Homebrew item the way you would for anything else. If you want it to have sentience, you may want to make a copy of an item that already has sentience or stats like you want (e.g. Moonblade).
In the description section, you can describe the sentience. Again, use an item, like Moonblade for inspiration/guidance on how to phrase things or how to style the Sentient portion in the description.
Sentience is just describing personality of the item. It is not the idea that the item acts of its own accord. However, you could state, as part of its sentience, that it can. Though every Sentient item I've ever seen just says that it convinces the wielder to do something, that it does something on its own. If it could, it probably wouldn't be an item or weapon. Like Tom Riddle's Diary - it couldn't physically do anything or go anywhere, but it could convince people to do things for it.

What do you mean? There’s nothing special required mechanically for a sentient weapon, it’s mostly just text on the description.
