Using Edge browser on both PC and Surface tablet. Custom character images / avatars issue as well (not showing).
I've tried opening the Edge Dev Tools (CTRL-SHIFT-i) as suggested and right-clicking on Reflesh > Empty Cache + hard refresh... but doesn't work.
I've taken a screenshot of the Edge Dev Tools details (image above); (right-click and open in new tab to zoom)
There's a "Tracking Prevention blocked access" yellow caution error message for each custom character image, as well as a red X non-access to the images' hyperlinks.
I'm not computer savvy enough to know what this all means.
Update: Added dndbeyond.com as a tracking exception in the Edge Tracking Prevention settings... resolving the Tacking Prevention blocked access yellow caution errors, but not the blocked image hyperlinks. So still no custom images loading up. Now, there's a 400 error message for the blocked images in the Edge Dev Tools window...
2nd update: right-clicking on Reflesh > Empty Cache + hard refresh... didn't work, but clearing image cache and browsing cookies via the Microsoft Edge sidebar did[ ... upper right three dots > History > three dots (more options) > Clear browsing data > (check off) Cookies and other site date + Cached images and files]... but only for the My Characters page. Custom character portraits still missing on the My Campaigns page, or when using/loading the digital character sheet.
3rd update:Well, that didn't last long. Images are gone again after clearing the cached images and browsing data a second time (even after restarting browser).
I don't use chrome; I use Edge and it occurs, whereas when I switch to chrome it doesn't occur.
The cookies are browser specific, so your least used browser will be fine. The only way to get rid of the problem is a hard cookie delete as described earlier.

So, instead of clearing browsing data in Microsoft Edge (cookies, image cache, browsing history, download history, all data from previous versions of Microsoft Edge) for just "Last hour", or "Last 7 days" or "Last 4 weeks" (which didn't work), I've selected "All Time".
Lo and behold, it worked.
Empty cache + Hard page refresh wasn't enough. I needed to basically clear my browser of all existing cookies and cache.
Hope that helps other people, and hope the solution sticks for a while this time.

The combined size of all cookies from dndbeyond.com is enough to cause this problem. So I just deleted another large sized cookie that looked like it wouldn't directly impact the site. The other day I did get an update on the support ticket the I had in on this that the team has been working on on the site cookie settings, specifically the length of time cookies persist.
Since installing the browser extension EditThisCookie and permanently blocking the Ratings field, I have not had this issue on my home or work PCs. I have noticed my phone's Chrome browser is missing images sometimes. Now to see if I can block that field on my phone too.

As a note, the EditThisCookie solution worked for me, but I had to re-upload all of my affected character portraits. When I followed all of the instructions (including a hard refresh and logging out and back in), several of my portraits were still solid black. However, I re-uploaded the portraits and they seem to be staying (at least for the moment). Thank you all for the helpful resources!

I have since removed the exception that blocks that cookie, and things have gone smoothly so far. I'll update later when things start disappearing again, and reapply the fix.

I still have it blocked - if things keep going smooth for you let me know it'd be nice to know if this is fixed. I haven't had the loading issues you were experiencing yet.
If this is indeed the full picture (pun intented) fix your cookies ffs. There is no universe in which having to install an extension to disable your abuse of cookie in order to prevent your user uploaded image component to be reachable. I am amongst those for whom deleting cookies and history does nothing. Seem to be provided with enough right when logging back in with my first view requests.
Just now works for me again. Hope this is because it was fixed, not just because something random. If it was fixed: Thanks a lot! May character art is important to me. So much more satisfying with it having returned.

You could use kiwi browser and load the normal desktop extension. Kiwi is based off chrome and works with chrome desktop extensions.

It was Cobalt-token, it was about 900 bytes.
As a note, the EditThisCookie solution worked for me, but I had to re-upload all of my affected character portraits. Also have this issue as of today. Clearing cookies didn't work. Please fix!! This is really frustrating!
Same issue here
The last time this happened; the "edit cookie" solutionw orked... This time it didn't.
I resorted to simply deleting the "ratings" cookie, which worked for ONE image... And promptly didn't anymore on another character sheet.
Browser: Firefox v100.0.2, 64-bit, PC.
UPDATE: did the "EditThisCookie, block ratings" solution... didn't work... did NOT work...
And a new discovery; some of the portrait frames don't load now either... funderful.
Still experiencing the issue with uploaded images; but the default D&D Beyond seem to be back.
Double-checking: Re-uploading/refreshing/scaling images down to the 150x150 size etc. etc.: Still broken.
