Is there a way to add lycanthropy via the character sheets on D&D Beyond? Like, I can manually adjust things, but the sheets are pretty good about being smart and I wonder if I'm overlooking a feature.
What are the mechanical effects you are applying to lycanthropy? One common way to handle this stuff is to put the effects on a magic item and then equip it when you want the effects.

Lycanthrope works well as a feat. Hence, also it works well as a custom race (where Tashas will allow a custom race: being +2 any ability, plus 1 feat, plus either darkvision or any one proficiency).
You can also design Lycanthrope as a Wondrous Item, a magic item, with a custom spell. Then simply activate the magic item to represent a transformation.
The Lycanthrope can simply be access to two alternate forms. I suggest any transformation requires 1 minute, but otherwise no other limit. Perhaps during a full moon, the transformation is involuntary. • Wolf. As a normal wolf with a 1d8* bite and Perception advantage if smell or hearing, and good speed 40. • Werewolf. Essentially a bipedal wolf, with 1d8/1d8* claws, and Intimidation advantage, and Darkvision.
Personally, I think that Shifters are a much better alternative to Lycanthropy. Even when it comes to good Lycanthrops, it’s still a curse that destroys the personality. Lycanthropes aren’t as good to role play as they have a fixed personality. The Bear is always going to be the courageous animal protecting the wild while the wolf will always be the Monster killing inocents. It shouldn’t be for PCs.

Completely agree. Even if you are going for the tortured soul aspect of the curse, were-touched/Shifter is a good base to work off of.
You just need to establish some triggers for the curse, like extreme emotions or the full moon. When one of these occur, you need to be make a save to resist a beastial compulsion (like making an attack at the closest creature), or involuntarily shifting.

Yah, D&D Beyond doesn’t have a template select option at the moment.
It might get it in the future at some point, since they’re developing the virtual tabletop options further.
You can see from my avatar that "I ❤ wolves" would be a good T-shirt for me, and I'm also disappointed. I just made a character, and when I looked for lycanthrope or even werewolf stuff, I couldn't find it anywhere!!!
There is a third party setting - Grim hollow that has a lot of cool rules on how to apply lycanthropy. It might not work with ddb sheets - but might give you some good ideas if you are willing to invest in it. There's a lot of good material in the setting book for darker/horror type games as well.

Depending on how lycanthropy is transmitted in your game you may need also a poison or disease addition to your bit attack.
I would also have some limitations on what types of equipment you could use in your were form as well as potentially require shape changing equip so it is not destroyed.
The thing to keep a eye on is balance vs other PC's and maybe even other forum builds to see if it is out of wack.
One thing I have seen people talk about trying to do in the past is try and say lycan is = to x magic items and make a formula for it for balance reasons. But I have not been impressed with the ideas I have heard or that have been laid out.
Honestly, they really shoulda added in rules for lycanthropy/therianthropy, when they made Van Richten's Guide. Best I can recommend is reflavoring the Barbarian's Rage, or working off of the Blood Hunter's Order of the Lycan subclass.

lycanthropy is one of the oldest curse in DND how the hell is it not in here so you can add it at a click of a button?

In the past people have created a magic item with the abilities the lycan had and toggled it on and off when needed.

That is what I did, to reset base AC, STR, etc for someone who is a werebear. It works well.

Off the back of this would it be possible to create a dragonborn who as a child was effected by a lycan
