Belthomias tries to sweep the Seer's legs as it steps back but mishandles his glaive somewhat. One opportunity missed means another one gained, and the ranger comes to Quadere's aid. He feeds her a potion and calls her back into the fight. (Quadere heals for 4 HP.)
Kandalou does his best to be useful now that the Hulk is down, but has to settle for fighting back the Far Realm's influence on his mind.
Quadere stands up and put some distance between her and the Seer. With her left hand, she then calls forth her spiritual weapon once more. The floating armament swings down immediately and adds a small bruise to the seer's back. (Seer takes 9 force damage.)
The cleric's right hand tries to summon a flash of sacred flame to add insult to injury, but the Seer avoids it easily.

The mind Flayer tries to make a meal out of the surviving amphibian.
-> Tentacles vs. Giant Toad #2: Attack: 13 Damage: 13
The illithid finally get a good hold of the toad's head, but the damage is enough to send the summoned creature into the ether. The brain-eater looks disappointed at its disappearing meal. (Giant Toad #2 takes 24 psychic damageand is DEFEATED.)
The Ulitharid [Tooltip Not Found].
At this point, the rrakkma feels marginally better about their presence in the Far Realm. Whether this is a cost-less boon, it remains to be seen. (The DC for the initial WIS saving throw is now reduce to 7.)
NOTES: Everyone should begin each of their turns by rolling a WIS saving throw {DC 7}. If you fail this check, you'll also need to roll a d10. Also, the map should be self-explanatory following my description above, but let me know if you have any clarifying questions.

Keeping up the pressure, Quadere sends her spiritual weapon after the seer while falling back herself to AT-45. Once there, she finally gives herself some much needed medical attention (3rd level), healing herself for 12 and close some of her most pressing wounds.

Despite having the Seer within his reach, Belthomias feels the need to pause for a few seconds before being able to refocus on the battle.
Just a few steps away, Quadere takes a moment to heal herself a bit.. She simultaneously commands her spiritual weapon to move and attack, but it doesn't swing hard enough to hurt the Seer. (Quadere heals for 14 HP.)
Kandalou recovers and spots the wounded Mind Flayer in a nearby corner. The fighter approaches and tries to kill it with two swings of his blade. The first one chops one of the ilithid's arms clean off, while the second opens a gash in its breastplate before cutting into its chest. (Mind Flayer takes 24 magical slashing damage.)
Kandalou raises his sword again, trying to finish the job but, before he can bring his weapon down, an arrow whizzes past him and sinks into the ilithid's forehead. Vishnu whistles, signaling they have no time to waste in dealing with the Seer. (Mind Flayer takes 29 magical piercing damage and is DEFEATED.)
The light emanating from the GIth Capacitor becomes blindingly bright for a brief moment, as the Ulitharid continues to [Tooltip Not Found] it is sucking in the strange energies of the Far Realm. You can all guess this is a sign that the tentacled beast is not far from reaching its goal.

Noticing that the tide of the battle is turning and that it is now alone on this side of the energy barrier, the Seer points its grotesque staff to the barrier itself and appears to drain some of its energy back into his weapon. A small crack forms on the barrier, but the Seer's staff now emanates its own sickly green light.
Then, it tries to warp the space around Belthomias and him send away:
-> Belthomias' WIS saving throw {DC 19}: 17
The ranger suddenly disappears, only to pop out of thin air right next to Vishnu {AS-50}. The seer then puts additional distance between it and its foes by walking west {AN-31}.
