Gender has always been a fascinating subject to me, and in D&D there are so many things you could play as genders on the spectrum!
I already shared about one character, but another one that I have is an androgynous bisexual(some flowering plants are like this) sentient rose vine! (This was a Gamma World character!)

Oh yeah. My character periodically comments about how strong and brave and pretty the other PC is (the other Hin woman is a Paladin with high Str & Cha). One time in one of her terrible attempts at flirting my PC thought it would be romantic to make a giant chalk drawing of the other PC’s face on the ground and drop the party’s tiny hut directly on top of it so everyone would sleep on a 10’×10’ image of the Paladin’s face. (You know the scene in Raiders of the Lost Ark with the student who wrote on her eyelids so every time she blinked it would say “luv you” to Dr. Jones? It’s almost like that. 😂😂) Of course I PMed the other player directly to make sure she was okay with everything and set up boundaries, and I periodically check back in to make sure everything is still okay. Consent, consent, consent. Right?
I usually just decide what my PC's orientation is based on if it fits their vibe or my vision in my head of them. Most of them are straight tho. He only let's his friends call him "he" n such, and is somewhere on the ace/aro spectrum, since I don't really see him being a romance/sex option tbh if the players tried that route. I have a bi wizard/bard dude, with who I was mostly thinking "If I played him in a duel he would 100% just hit on his enemy the entire time to knock them off their game" the whole time in making him. His name is Klark. He's a clerk at an adventurer's guild. I have a bi/poly (haven't decided) bard named "River Powers" who can change their sex and appearance to the opposite gender whenever they want and can hop universes, planes, timelines, and can time travel. So if I ever wanna drop them into a CoC/Traveller/5e/Starfinder/etc, game the players can be like "I KNOW THIS ***** WHY THEY HERE, WHAT CHAOS ARE THEY CAUSING?!?!?" and it makes sense.
I made a lil' table for myself in case I ever needed to quickly decide on the way an NPC leans. I looked up the distribution of orientations irl and put it in a d100 table accordingly. I'll stick that here in case anyone wants something similar:

Hmm.... most of my characters just go by their birth genders, the only character I guess could be counted as gender queer is my blind elf, Silver! He's very physically androgynous and is basically alright with whatever gender people see him as initially, though if asked, he will consider himself a guy. Also, Silver is another one of my bi characters.
One of my characters, Arthur, was a human man who ended up stealing the body of a vicious aasimar bandit queen. He'd resorted to using a psychic artifact attuned to the Far Realm that the bandits had taken from an abandoned mind flayer colony. Body snatching is obviously not a very heroic thing to do, but Arthur doesn't especially feel guilt about it, and the alternative would have meant his death.
Arthur still considers herself mentally male, but responds to feminine pronouns. Partly because she genuinely does seem to enjoy life as a woman more, but also because explaining her backstory to everyone she meets isn't exactly time efficient. Her sexuality is far more straightforward as a lesbean, since she was a straight man before. One thing she absolutely refuses to change, however, is her name. There's in-story reasons why, but mostly it's my excuse to have a red flag in why a woman has a man's name.
