Every DM has their own method for creating an information doc for their players, if they make one at all. I'll share my own template, and I'd like it if other could share theirs as well, so that everyone can get new ideas from each other.
Overall Backstory
4-5 paragraphs on recent events that affect your campaign
Character Backstory
The backstories of each character for easy reference; can be filled out over the course of the campaign
Campaign Backstory
The event(s) that set the campaign in motion, as well as the reason the characters chose to follow the campaign
Class Roles (Optional)
The role that each class usually plays in your world
Racial Roles (Optional)
The role that each race usually plays in your world (ethnicity, society, discrimination, etc.)
Important Things
Any important people, objects, or places (1-2 paragraphs each)
Organizations
Any major organizations in your world, arranged in this format:

I made three 1-page handouts to start -- a races one-sheet, a deities one-sheet, and a "Roman society" one-sheet (we are playing in the Roman Empire). Then I gave them a map that showed the empire and its surroundings and listed where each race originated (they live in many places now but each race has a "homeland").

I like to do in world lore dumps - a page from a diary, an invite to an event, the rules of a contest, a religious reading etc. Something that feels tangible the characters would come across.
So when a player and I bashed out how Hags & Witches would fit in the world and worked out the mechanics of how they related to eachother, how a new hag is made, what they thirst for etc I wrote a traditional Camordian fairytale. Maybe it’s a true story, maybe it’s a warning but within the fairy tale are all of the key facts about Hags & Witches
