Another thing to consider as a DM is that you can roll dice for other things than just the hard & fast attack rules.
For example, I have a oneshot written up where the caravan is attacked by bandits. If the party is away, which I anticipate they will be, then I have a d20 roll which determines who lives and who dies in the caravan. There are a lot of superfluous characters added for fluff, so their living or dying is not intrinsic to the plot, so they each have a different chance of survival. I roll high, few people die. I roll a 1, and everyone not plot-critical dies.
You could implement something yourself for this - the BBEG, if they get the NPC in position, you roll a d20 and if it's under a certain value the BBEG kills them immediately, if it's over a certain value they pause to gloat, etc. Don't tie vital plot points to dice rolls, becuase if you fail the roll, you'll just be looking to see what you can do to fix it. Might as well just say "he stabs him". Attack rolls are for the uncertainty of combat, not for the plot.
