Hello, I have translated the tarrasque from D&D 3.5 into 5E. I have also added one or two things to it that were not there before. Over all though, it is what I would say is a 80% accurate translation into 5E. Please, do tell me what you think of it!

Magic Beast.
The tarrasque’s natural weapons are treated as magical for the purpose of overcoming resistance and immunity to nonmagical attacks and damage.
Augmented Critical.
The tarrasque’s bite attack threatens a critical hit on a natural attack roll of 18-20, dealing triple damage on a successful critical hit.
Frightful Presence..
The tarrasque can inspire terror by charging or attacking. All creatures within 60 feet of the tarrasque when it takes the attack or rush action must succeed on a DC 19 Wisdom saving throw or become sick with fear. Any creature that fails the saving throw is poisoned, remaining in that condition as long as they remain with 60 feet of the tarrasque. This does not affect creatures that are immune to the frightened condition.
Regeneration.
The tarrasque regains 40 hit points at the start of it's turn. The tarrasque regenerates even if it fails a saving throw against a disintegrate or power word kill spell. If the tarrasque fails its save against a spell or effect that would kill it instantly (such as those mentioned above), the spell or effect instead deals nonlethal damage reducing the creature to 0 hit points. The tarrasque is immune to effects that produce incurable or bleeding wounds, such as mummy rot, or a vorpal sword.
The tarrasque can be slain only by reducing it to 0 hit points and casting a wish spell to keep it dead.
If the tarrasque loses a limb or body part, the lost portion regrows in 1d6 minutes (the detached piece dies and decays normally). The creature can reattach the severed member instantly by holding it to the stump.
Alertness.
The tarrasque's incredible senses are always on the lookout for food. The tarrasque cannot be surprised while it is conscious, as well as gaining a +5 to initiative rolls. In addition, unseen creatures do not gain advantage on attack rolls as a result of being unseen.
Unchanging Hunger.
Immutable Form. The tarrasque is immune to any spell or effect that would alter its form. Further more, the tarrasque is immune to any spell or effect that would alter it's stats.
Reflective Carapace.
The tarrasque’s armorlike carapace is exceptionally tough and highly reflective, deflecting all rays, lines, cones, and even magic missile spells. There is a 30% chance of reflecting any such effect back at the caster; otherwise, it is merely negated. Check for reflection before using the creatures magic resistance.
Magic Resistance.
The tarrasque has advantage on saving throws against spells and other magical effects.
Actions
Multiattack.
The tarrasque attacks once with it's bite, twice with it's horns, twice with it's claws, and once with it's tail.
Bite.
Melee Weapon Attack: +19 to hit, reach 10 ft., one target. Hit: 36 (4d12 + 10) slashing damage. When the tarrasque successfully lands a bite attack, it can then attempt to grapple as a free action. If it wins the grapple check, it restrains the target and can try to swallow the foe the following round.
Horns.
Melee Weapon Attack: +19 to hit, reach 10 ft., one target. Hit: 16 (1d10 + 10) piercing damage.
Claws.
Melee Weapon Attack: +19 to hit, reach 15 ft., one target. Hit: 28 (4d8 + 10) slashing damage.
Tail Slam.
Melee Weapon Attack: +19 to hit, reach 20 ft., one target. Hit: 24 (3d8 + 10) bludgeoning damage. If the target is a creature, it must succeed on a DC 20 Strength saving throw or be knocked prone.
Swallow.
The tarrasque can try to swallow a grappled opponent of Huge or smaller size by making a successful grapple check. Once inside, the opponent takes 19 (2d8 + 10) points of bludgeoning damage plus 56 (16d6) points of acid damage per round from the tarrasque’s digestive juices. A swallowed creature can cut its way out by dealing 50 points of damage to the tarrasque’s digestive tract in one round (AC 15). Once the creature exits, muscular action closes the hole; another swallowed opponent must cut its own way out. The tarrasque’s gullet can hold 2 Huge, 8 Large, 32 Medium, 128 Small, or 512 Tiny or smaller creatures.
Rush (Recharge 10).
The normally slow moving tarrasque moves up to 150 feet.
Legendary Actions
The tarrasque can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature's turn. The tarrasque regains spent legendary actions at the start of its turn.
Move.
The tarrasque moves up to half it's speed.
Attack.
The tarrasque makes one claw attack.
Chomp (Cost 2 Actions).
The tarrasque makes one bite attack or uses it's swallow.

Ah wonderful ive always said that the Tarrasque should be as strong as it was in 3.5 but never knew how to convert it properly.
One thing i may add though in my games is maybe adding back its damage reduction or a damage threshold instead.
