When you realize you're doing too much: Signature.
Flurry of blows just says you get two attacks for your bonus instead of one. It says nothing about them both having to be directed at the same target.
Vondal grins as he winds his axe back around for another upward chop towards the chin. "I believe you meant -- Like. THIS!" The blade cuts across the creature's shoulder. The monk then follows through with an elbow to the wound. The orc growls in pain. Vondal then jumps over to join in on the mashing of the (black) orc's knee with a meaty fist, which lands with a pop.
The (red) orc takes a swipe at Vondal as he moves out of melee range. (greataxe attack: 24, damage: 9 slashing damage) The beast chuckles as his axe cuts a gash down Vondal's back.

Seeing the Ogre giving Karl some trouble, Nif puts it in his focus. He reloads his crossbow and fires at the Ogre. He takes a step forward to make his shot and his foot slips in something, sending the giant bolt clattering against the rock, but he is able to keep from falling.

I'm assuming the cure wounds is on yourself, Zander. I see you ticked off the necrotic shroud on your character sheet, but you didn't tick off the two sorcery points or the level one spell. Don't forget.

The (red) orc takes a javelin and throws it at Vondal. (javelin attack: 22, damage: 5 piercing damage) Distracted by Zander's transformation, the monk doesn't realize he needs to duck, and the projectile catches him in the back of the shoulder.
Both the (black) and (yellow) orcs swing wildly at the scary creature which is Zander. (greataxe attack: 13, damage: 14 slashing damage & greataxe attack: 24, damage: 11 slashing damage) Despite their fearful, wild swings, both creatures strike Zander again. He goes unconscious. (Nothing I read says necrotic shroud ends at unconsciousness, so I'm going to say he's still altered, even though he's unconscious.)
The (pink) orc swings again at Karl. (greataxe attack: 17, damage: 14 slashing damage) After knocking down the (orange) orc, he catches his weapon and spins it upward, deflecting the (pink) orc's attack.
The ogre brings his tree trunk-like weapon down at Karl again. (greatclub attack: 18, damage: 8 bludgeoning damage) After deflecting the (pink) orc's attack, Karl brings his shield around almost too late, but is able to smack the ogre's weapon away.

Not sure what's going on with the manipulated dice rolls, but you were reducing 5 damage and 5 is your modifier, so you didn't even really need to roll. I'm glad you did because it helps catch my attention, but I'm saying it doesn't matter if you did manipulate the dice rolls, the result is the same: the 5 damage is negated.

(@DDP: Yea, I don't know what's going on there. I put it in right the first time, but then when I hovered over the dice roller, it said it was 4d6 at first, like I had used part of the ability score roller. :/)

The monk then looks to his right and, seeing the (pink) orc still harrowing Karl, moves across the cave floor like a blur, jumps up onto the cot, lands on top of the crumpled (blue) orc (I4), and brings a foot in to the outside of the (pink) orc's legs to try and snap it at the joint. The orc growls in pain. Whatever momentary glee there may have been proves fleeting, as one glance at Zander from even the corner of his eye leaves Vondal chilled and wide-eyed once more.
