You don't add your proficiency to damage rolls - only attack rolls.
The reason your Battleaxe and Dagger are +5 to damage and the rest are +3 is because you have the Dueling Fighting Style which adds +2 damage as long as you are only wielding one weapon (and one-handed - hence why using the battleaxe as versatile is +3). If you dual wield them you would not get that bonus.

I have a character who's Modifiers seem to be incorrect.
He has proficiency in both Simple and Martial weapons. His +3 Strength and +2 proficiency bonus should give him +5 on attack and damage.
This is true for his Battleaxe but for the Great axe, the attack bonus is +5 but the damage +3.
Also, the Battleaxe, when two-handed, only has +3 damage.
The Longbow is also showing the proficiency bonus in attack but not in damage.
Am I doing something wrong?
