I think Underdark campaigns would be more served with ecology than player options, beyond maybe some backgrounds like miner or spelunker or "under dark" equivalent of the outlander.
I mean I think the point of the character subclasses in 5e is not to bind them to particular geographies, so they can be used anywhere. At most an Underdark druid, though Circle of Spores would easily be found in the under dark, something more tied to subterranean elements. Maybe an "under dark" Cleric domain.
Feats for tremor sense and blind senses.
But the bulk of it should be rehashing how to do three dimensional subterranean. realms ... which a third party publisher is apparently putting together.
I know folks say player options are essential to WotC business model because more players than DM = broader market. I don't know how well test or proven that theory is. Regardless I think a book that would give players knowledge on how to plan for under dark expeditions or existence might be just as valuable to a player as mechanical options to the character sheet.
It seems two subclasses is WOTC norm, so I'll put in two for classes that'd need them down there: a druid that works with stone, and a fiend/stone sorcerer as well.
Lots of monsters. All the monsters.
Maybe an optional lineage that players can customize - "demon-born"? Demonic tieflings?
I'm down for an Underdark sourcebook with the caveat that Menzoberranzan should not be the only Drow city discussed. I don't remember if it was the 1e or 2e Forgotten Realms sourcebook that said Menzoberranzan is actually one of the smaller Drow cities and yet it seems to be the only one that's ever written about...
A stat block for Lolth would be cool...
I could also see them redoing the D-series modules, Queen of the Demonweb Pits, and/or Night Below and building a sourcebook around that... At the same time, exploring non-Drow stuff is important too. There's plenty of aberrations and whatnot to explore...

Quaggoth PC race (I like the Derro option too!)
Subclasses:
Demon Druid- Wildshape allows you to wildshape into certain fiends
Gloomrager Barb- While in darkness/dim light you get additional stuff from rage and maybe terrify those in darkness while raging
Demonic Bloodline Sorcerer- Mostly covered by Shadow sorcerer but maybe have some ability to pull directly from Faerzress to upgrade spells
Magic Items: Drow weapons, Derro magic items, Items made from creatures found in the Underdark
Spells: Buff/Debuff spells that work better in darkness/dim light
Faerzress based spells

Or is that a fiend with druid levels in a Circle of sin? wildshaping into a humanoid could be an interesting thing.
