Wish is already cast At Will, but the components are a coin and a water well. Upon "cast", it's a 5d100 and a 5 results in a curse as a perversion of the wish while a 500 results the wish as intended (regardless how it's worded).
6-250 compels the character to toss another coin into the water well. If the character has no other coin, the least valuable item in possession is thrown in (which is often the standard set of commoner clothes). If the character carries no further possessions, the character jumps into the well. Regardless what goes into the well next, there is no further effect. (EDIT: ...except the character screaming on the way down if applicable.)
251-499 results in any passerby giving a disapproving opinion of tossing coins into water wells.

If you're a Chain Warlock, and you chose imp as its first form, then cast Find Familiar again, you can turn an imp into a flying monkey. So, yes, in a way.
(Also, polymorph would work as well.)

Any player that does anything not expected in the campaign immediately dies for no reason. (If the players are supposed to go into town but one tries to go to the docks, that character dies as soon as the character makes the first step in the wrong direction.)

If a player is about to make the character do something extraordinarily stupid, the player must have the character pass a DC10 INT check to have the DM call the player an idiot OOC. (Yes. Pass the check to be told it's a stupid idea.)
