I see the modifier you're using that is causing the issue.
The modifier for dealing additional damage should be "Damage -> Additional". If instead of a die amount it is a set amount, like an ability modifier, that should be written in the "Details" section of that modifier. You should not use the ability modifier field at all for the "Damage" modifier type. That field is used for other modifier types.
The effect you describe here is not one that will be displayed as you're hoping on the Attacks/Actions pane of the character sheet, it will instead just appear in the item description/notes. I know that is not what you would prefer, but entering it this way should cease the Beholder errors.
It's important to note that the system is built to support the official magic items from the 5th Edition rules for D&D, so it may not always allow for the exact permutations you're aiming for.
(Unrelated... but a helpful note that the Bonus - Melee Weapon Attacks will apply that bonus to all melee weapon attacks, not just this weapon. The only modifier that changes a weapon's attack/damage is the "Bonus -> Magic" modifier, which is used for all of the +1, +2, +3, etc. items that exist in the official rules. This changes both the "Attack" and "Damage" modifiers by the amount chosen.)

(made a few versions) any idea? if it does add, it then gives the same error when you try an dclick it in the action list on the character.
