I can recreate the issue with Chrome for Mobile, Firefox Mobile, and the DDB app. All of the above produce the error page even after clearing the app cache\data.

That WILL log you out of the site and mess up some settings, but the bug should be dealt with... At least: for as long as it takes to manifest again.

Removing all ddbSiteBanner:######## cookies...DID fix it. I'm not sure why the initial cookie clearance didn't work, it definitely cleared my login details.
I'm having the same problem, but it's with profile pics. I can't see my own profile pic but I can see other people's. I already tried clearing browsing data and cache, and no luck.

Is there a plan in place or something to resolve the issue in the works? It would be great if we could get some kind of word back about it from someone on staff,
i was just having this problem... and it was getting worse. first one custom character image, then most forums avatars, and then all but two of my character custom images disappeared. app was still okay, even reflecting the character image changes (made in browser) from when the black boxes first began. i followed the firefox "about:preferences#privacy" cookie delete advice above. had to do it twice for some reason. just for good measure i checked Task Manager and found three instances of MS Edge browser running uninvited. end task. restarted firefox, and... poof! back up and running again!

Unfortunately: it looks like this is the second solution that no longer works despite the problem remaining across a lot of browsers... Good job "fixing" the solutions guys: now try fixing the problem!
Using the debug tools (see How to see HTTP Request Response Headers in Google Chrome Browser | Code2care), I found the request header for this picture of Ais Wallenstein. The total size of all cookies on Edge with error 400 was 6828 bytes. On Chrome, a browser I seldom use, it was 5837 bytes and successful. Neither include the rest of the payload of the header, which in the edge case must be 1364 byte.
Both cookie sets had both the Ratings cookie (2222 in chrome, 2246 edge), the next one was cobalt-token (both browsers it was 1559) and the third largest was Sailthrough_content (144 vs 528), which also had the largest delta. Edge had 519 bytes more in cookies not present in Chrome, and 492 bytes in growth in other cookies (again mostly in Sailthrough_content).
"...Request headers today vary in size from ~200 bytes to over 2KB. As applications use more cookies and user agents expand features, typical header sizes of 700-800 bytes is common."
The entire header is the problem not a single cookie, and there are three cookies that represent 68% of the cookie payload. The cobalt cookie is required; its the cookie that tells dndbeyond who you are (found that at https://www.youtube.com/watch?v=9PjPkugkF1o ). Sailthru tracks recent page views (https://getstarted.sailthru.com/welcome/implementation/personalization-engine-setup/) leaving us with the ratings cookie which is about the same in both cases. But the total aggregate in the case of an error 400, is what push it over.
I outlined some server side fixes before at: In post 30 . However, unless people go into debug mode and see if the images loaded are error 400, it wont be clear if there is more classes of issues here. It not yet clear to me what ELSE is in that header; because 1384 bytes alone is larger than the average request header.
Hoping this gets a proper web developer to comment on this; I just claim to have slept in a Holiday Inn once.

So I don't use Chrome on Dndbeyond, I use Edge. So digging into the debugger found an image that doesn't load in Edge. The error:

On Chrome, the image appears as expected. This is again the same issue, the header is too large. See Here's How To Fix Request Header Or Cookie Too Large Error (techgearz.com) write up for general coding. And in prior solutions removing the cookies was the most common solutions.
