See how the +0 next to Perception is blue instead of black? That means you've manually modified that skill. Click on it to get the sidebar, click 'Customize' to expand the customization options, and delete whatever's there.
Well yea, that is because I was trying to fix it, force it, on my own first.
I have already removed any mods/customs from it.
But it DID show up as 0 Wis mod to both Perc & PP BEFORE I even tried that.
Any more thoughts?

It was the cause. You've removed. It's now fixed and showing the correct number.

As title stated; my Wisdom mod is not being added to Perception or Passive Perception.
All other skills - including those using Wisdom - are being calculated with the correct Abil mod (as well as correct Prof bonus where applicable).
New Dhampir lineage.
Someone take a look please?
