Unless you want to publish this and want it to work like an officially implemented class would you can just add some text to the subclass or race or whatever it is that'll provide this and manually add the proficiencies to the character sheet. No need to complicate it any more than really necessary.

Making a homebrew...
At 3rd the character gets a choice of 2 of the following 3 proficiencies: acrobatics, performance, or one kind of gaming set.
Is this possible?
