IMO, the breakdown of your stats and the issues you're facing, I'd grab a shield and talk to the DM about +1 or +2 studded leather. This will take your AC to acceptable levels for standing front line at your level, with, as mentioned before, Shield of Faith and the feat bonus to boost it as you see fit. An increase of 2 or more to your AC will likely have a big impact on how survivable you are in front line combat. As well, if there is any way to force disadvantage on foes swinging at you (long shot there) that also goes a long way.
Weaponry I'd stick with the Rapier. It's the best damage a Dex weapon is going to offer, so looking for uppity versions (Flametongue, +1 or +2, and so on) will help keep your damage where you want it. If you take enough Rogue for Evasion, then fine, otherwise, it's a feature of the Shield Master feat that seems to come up (in our group) enough to be worth having if it's viable. Our caster will freely throw AE Dex-based save spells while the Fighter and I (Monk) are well within their range.
