Really for most of tier 3 the sheer cost of a 6th level slot is a significant offsetting factor; if you use it during a day you’re expecting combat, that’s a lot of damage you’re passing up on. Plus it’s not exactly subtle, and transitioning back means you’re unable to act for 1 minute and the only protection you have is resistance to non-magical PSB; for infiltration purposes the phrase “sitting ducks” comes to mind. As for bypassing random traversal encounters, that’s rather the point.
Exactly. I used this spell exactly once on my Druid (level 16 atm). We needed to travel a long distance out of a desert area we were adventuring in, had lost our horses and camel in a sand storm earlier and there really wasn’t anything else we needed to do in the area except travel. Had to long rest to prepare it and once done traveling unprepared it and never looked back.
a brief tangent but I agree it says that in the spell casting section but the listing for the incapacitated condition doesn't, it just says "INCAPACITATED: An incapacitated creature can't take actions or reactions". So I was saying the entry for the Incpacitated condition should have a reminder of that section from the spell casting section.

UA 1 for One D&D / 5th 2.0 / whatever did have it there, so hopefully the new PHB in its 384 page glory might.
