So, I decided on impulse to add the mob to my campaign. How do I run it? They are using a fancy hotel as a front, and the boss is the grandmother of one of the NPCs that travel with the party.
A mob boss rarely engages in combat themselves, rather they order their henchmen or underlings to enact violence on their behalf. I don't know what level your party is at, but the mob boss can be either someone who is so powerful and intimidating that other criminals serve them, or the boss has a high charisma, influence and wealth that they command powerful criminals to work for them, like the Noble.
Because this is a grandmother, perhaps you can use one of the Hags as a stat template. They can also have a number of goons like the Berserker, or Bandit Captain, Changling, Gladiator, Goblins and Hobgoblins, Pirates, Illusionist, Werewolf, Martial Arts Adept, Giant, etc.
You can look at different media to see examples of how mob bosses work. A recent example would be Odin from God of War Ragnarok. Another example would be Silco from Arcane. Mob bosses usually aren't motivated to conquer cities and kingdoms, but rather to maintain the status quo and reap the rewards from a broken society.

Yeah, this is pretty much the best answer. Read up on the Z's and then reskin them to suit your mob campaign.
Possibly give your players the option to "call" the mob and have a mob member show up to assist them.
