I'm a DM that has been running a long run home brew campaign that has involved several instances of improvisation on my part. I introduced an NPC that has evolved over time and I've decided I want to make them the big bad of the campaign, something I've already given my players a bit of a glimpse of with some foreshadowing. I'm now struggling to decide what they are, is there something established or will I have to home brew a completely new monster?
So the character first appeared to be an elf but it was soon established that "she" is Fey and from the Feywild. Without out rambling on too much but to set the scene, my players have formed an uneasy alliance with a crime boss, who has this fey-elf lady stowed away due to her ability to produce a flower at will that can be made into a powerful opiate and is therefore the centre of this crime boss's drug empire (yeah it went a bit Breaking Bad.) My players have been charged with guarding the fey-elf and the building she is hidden away in.
On appearance the fey-elf is very "floaty" and child like, as if on her own little trip and not quite with it. Lots of smiling and staring as if looking into a world no one else can see (she also knew eveyone's names without having to be told, "just by looking into them") Imagine Drucilla in early Buffy but less murder-y. She likes to spend time swimming and bathing in a pool/spring she made in the basement of this building, magically crafting these flowers. She doesn't seem to know or remember who she really is and has been given the name Lilith due to the flowers she makes being similar to Lilies. All in all, at first she seems harmless and mostly innocent.
My players killed an enemy and had to hide the body in the basement with Lilith, who proceeded to drain what life essence was left in the body, leaving it a dessacated husk. A process which suddenly seemed to make her more lucid and aware for several moments before slipping back into her floaty self.
So my thinking is that Lilith is going to turn out to be some big evil Fey that has been exiled from the Feywild, a process that has made it forget what or who it is. I want it to be a "world ending" threat my players have to face. I'm not sure there is something pre designed in one of the manuals or guides but so far I'm struggling to find anything, so wondering if I'm going to have to home brew a new monster from scratch.
Any suggestions or ideas would be much appreciated.
Those would be REALLY big baddies, like fighting gods, a tough ask even for L20 characters... but if this archfey has been exiled and de powered, you can use that as an excuse to stat her at whatever level is appropriate for your campaign.
That sounds like an awesome idea, thanks for the info. Will deffo have to lower stats as players aren't quite ready for god levels of foe just yet, but maybe a race to stop her consuming enough life force from the world to re gain full power.

For making a world-ending threat, make her an exiled archfey! There’s lore on them at https://forgottenrealms.fandom.com/wiki/Archfey .
