The Ink of Coatl, 3 charges a day A serpent tattoo and can summon Arms of Hadar/ Evards black tentacles (maybe at higher levels, also thinking of the PC counting kills made with this ability to evolve it) in the form of snakes instead of tentacles.
Crossbow of Chains, 3 charges As a bonus action, you can cause a bolt fired from this crossbow to change into a chain that connects to any surface within 10ft of target. The target cannot take movement or reactions while restrained this way. Can make a strength check at end of their turn to break free.
Long arm of the Golem, +1 unarmed attack if made with off hand A stone gauntlet with a blue transparent stone on the forearm. Inside the stone the wearer sees a ever rotating spool of chain. The wielder may use a bonus action to fire their hand at a target up to 30ft away, which is connected by the chain to their arm to pull medium to small creatures 15ft closer. Alternatively if the target is large or bigger the PC would be pulled 15ft towards the target.
Plague Doctor, +1 Dagger, 3 charges per day A wicked blade that appears to be constantly sweating. As a bonus action when attacking with this blade the user may poison the target for 1d4 damage. Damage cause this way will convert to temporary hit points.
A Mysterious Egg A large mysterious egg with a stone band around the middle. The band has several gylphs around it. At the beginning 1 spell may be stored within the egg, making a glyph glow. More spells may be stored at higher levels. After a time the egg will hatch but the hatched creature will retain the stone band for spell storing and it will resemble a collar of sorts. The hatched creature will be bonded to it user.
This last one I made specifically for a certain PC and I have a creature in mind to hatch but presumably it will be at DMs discretion.
Let me know what you think or any small tweeks you might make? These are for a homebrew I'm working on at will be given as rewards to LVL 5-6 PCs. Meant to be their first magic items of the campaign. Just a little bit of fun for them and kind of personalized for my group.
The magical tattoo has 5 charges. While holding it, you can use an action to expend 1 or more of its charges to cast one of the following spells from it, using your spell save DC: Arms of Hadar (1 charge), Evard's Black Tentacles (3 charges). Instead of shadows and tentacles, these spells produce magically animated snakes.
The tattoo regains 1d4 + 1 expended charges daily at dawn.

The crossbow bolt transforms into magical chains which bind the target to a surface of your choice within 10 feet of the target. At the end of each turn the target must succeed on a DC15 Strength save to end this effect.
Mainly just format/templating closer to what's in the books. Its always easier to find an existing magic item to base the wording off of than try to cobble together the wording yourself. These are definitely fun items and I see a heavy chain/binding theme through most of them.
