How i choose monsters targets for their attack depends on a number of factor like it's nature or intelligence, how tactical or smart it is, its goal in a given fight, its relation toward the characters etc
Monster generally attack an enemy engaged with it in melee, at range it will attack the one that pose a bigger threat. Extremely raveneous creature such as ghoul may keep attacking dying target as opposed to threatening one. In case of multiple possible targets, i most often rely on randomness, rolling a die to determine who is the next target.
Depends; I sometimes use a dice to determine when there's multiple in a cluster and they don't have AoE. However, I've had players verbally challenge the big bad, and it makes sense if the baddie is egotistical. But I do try to use the randomness of the dice so that there's no "targeting" of specific players perpetually, and will roll these dice rolls in the open for my table to see., too.
