I am running with CoS with my players and we using milestones for level up. We just finished Death House, which tells you when to level up, but the rest of the adventure doesn't.
I know there are some guidelines in the beginning of the book but they are a bit too vague for my liking.
What are milestones you have all used when playing through CoS?
I gave my players a level anytime they found one of the important magic items.
Beyond that I kinda just guesstimated when they had completed enough story events to merit reaching a milestone. Just a few ideas are
When they leave a town (assuming they took time to complete most of the encounters/events associated with that town).
After they get their fortune read (only for one more early level, i'd say. Definitely push your players towards this encounter. I tried not to railroad my party into it, but it ended up with the party sort of aimlessly wandering Barovia while they tried to level up enough to face Strahd. The fortune reading gives them things to search for).
Fully/Mostly exploring the Amber Temple (this is a very dangerous dungeon and takes a few sessions to fully explore). I would say Aargynvostholt and the druid encampment on the hill are also good areas for rewarding levels if players take their time dealing with it fully.
Also, if it comes down to it, you can always have leveling occur in Castle Ravenloft as well.

There are milestones listed on page 6, though I advise requiring multiple milestones for higher levels so completionists don't hit too high level.
I started off running Strahd using XP because the players wanted to go that route. I am finding that milestone is much better so we are switching next session. I like the idea of leveling after each town or adventure and having the flexibility to modify as needed.
