1) If the Phase Spider is seriously injured, like when it disappeared
Khessa, seeing the Phase Spider in serious condition, decides to finish it off before it can do further damage. Her fingers intertwine a rapid dance, in concert with esoteric syllables of power, at the end of which three darts of pure magical energy shoot towards the target, changing direction when it tries to move and leaving it no escape!
2) If the Phase Spider seems to be healed compared to when it disappeared
Khessa, seeing the Phase Spider healed compared to when it disappeared, curses under her breath...
Aware of being among the best defended members of the group, the blonde arcane warrior rushes forward, brandishing her Greatsword and, to draw the attention of the supernatural predator towards herself, she delivers a powerful slash towards the monster's abdomen.
"There you are..." Theldrin mutters under his breath, not surprised to see the Phase Spider reappear. He quickly aims a Fire Bolt at the creature's abdomen area, then loads his crossbow.

Hoping to drive the spider away or to at least allow Khessa to get another swing at it, Xanrym gives the creature a withering glance and says, "I'm afraid you've only returned in time to go down with the sinking ship." Then he casts a spell to drive it away. Finally, Xanrym tries to he draws his spear, ready to fight off this monster again.
Bonus Action: Use Unsettling Words against the Phase Spider The Phase Spider suffers a 6 penalty on its next saving throw (against Xanrym's spell). The adjustment is reflected in the saving throw DC below. Action: Cast spell (LVL 1 slot) Dissonant Whispers, WIS 26 save or take 8 psychic damage and immediately use its reaction, if available, to safely move as far as its speed allows away from Xanrym. On a successful save: 4 psychic damage and no forced movement. Movement: If Xanrym is not/no longer adjacent to the Phase Spider, he moves back so that everyone other than Theldrin is between him and the spider.

Bite 2 against Syllen Attack: 7 Damage: 7 Piercing and 5 Poison.
If either Bite drops Syllen to 0 hp, he becomes Stable, and has the Poisoned condition for 1 hour. While Poisoned, he also has the Paralyzed condition.
You all are up. The Phase Spider looks really hurt, but is using it's last breath to try and take as many of you with it as possible.

"CORIANA!! Move!" shouted someone, maybe Xanrym, as Coriana stood, transfixed by the power of the tentacles. The sheer power of the unknown in the depths overwhelms her, as if the enormity of the weight of her pact was hitting her for the first time again.
Grab the chest and jump, then everyone can focus on escaping. Do it, Coriana, she thought, but she couldn't stop staring.
Her mind suddenly snaps to as the water rises. She jumps out of the hold through the openings and dives into the Nyr Dyv, her effortless glide through the water to the fleeing rowboat. She sees the party battling that damned ethereal spider, hexes it and strikes.
Now..
BA: Move Hex to phase spider. Eldritch Blast: AC 20, 10 force, 3 necrotic.

Action: Cast spell Vicious Mockery, WIS 14 save or target takes 4 (psychic) damage and has disadvantage on the next attack roll it makes before the end of its next turn.
