As soon as Karl sees Einar ready the hatchet and expecting that he may do something like he did. Karl smacks the Green Goblin in the head with 'The Rattle' muttering to himself "the direct approach is always the best one!"
After some moment of hesitation upon hearing the threat, Vondal's had begun to lower his bow on the goblin still within actual sight of the dwarf. Not so fully that he couldn't whip it up in a split second, but enough that he (or she?) could tell he was at least willing to listen. But whether at some point in transit for a better view of the hostage or well before, Einar and Karl spring into action.
Having caught none of the whispered conversation, or at most only the last vestige of it, a startled Vondal can only shout out, "WAIT". But it was too late. "You mutton-headed, cave-slinking -- MmmmGPH!!" He brings the bow and arrow to bear, and fire at Green to finish him off if he wasn't already dead. And if he is, he rushes up to Karl's back and fires a hopefully already weakened goblin orange.
Not sure if you wanted to consider Karl and Einar's actions as "Surprise" round actions, but if continue in the current order, then consider this a pre-post for when his turn comes back around. Because for his original one, it would've only been to move as far as H4.

The (orange) goblin's arm jerks with the impact of the attack, and the unsteadiness of the hostage causes him to topple forward and off the escarpment. The body lands at the base of the stairs (D7) with a thud and lies still.
As soon as Karl sees Einar ready the hatchet and expecting that he may do something like he did, Karl smacks the (green) goblin in the head with 'The Rattle', muttering to himself, "The direct approach is always the best one!" The (green) goblin falls lifeless to the floor of the cave.
Vondal squeezes by Nif (H4) and ...
Vondal, Green is dead. From H4, I'm going to say Orange has half cover. Black would have no cover. From G5, I'd say you could attack either goblin directly. What would you like to do?

Karl charges up the stairs to B8 and swings The Rattle at the Black Goblin (Attack 23) "and hitting it cleanly knocks it down the stairs where its broken body lies beside the unconscious old man
Einar draws his second hatchet as he sidesteps closer to the stairs, keeping his shield up and between himself and the goblins on the ledge. When sees an opening, he hurls the hatchet at the boss once more.
Action: Attack: 14 to hit goblin boss for 7 slashing damage.

If somebody hasn't already done so, Karl comes back down the steps, gives the Black Gobbo a good kick and says "that's for 'Bron - A Mate', prickly and 'ornery but still a mate" on the way past to checking out the old codger.
As Karl comes back down the steps, Einar makes his way up to check out the upper ledge, knowing that others are better equipped to help the old human.

Vondal is already at the old man's side, if not close to it by the time Karl comes back down. The whole time he is frantically fishing stuff out of his Healer's Kit(7/10 Now) to stabilize the old man, he mutters curses and comments in a familiar yet virtually unheard of language(Jamaican creole) that only Nif understands. And what he hears would be enough to almost make a sailor blush. Almost.
But when it gets down to it, he is focused in his work, both in turning the old man over, checking for any signs of an extended injury, and applying what gauze, salve, and pressure is needed to at least keep the man from dying in the next few moments. Only once the process is done does the Gold Dwarf let out a sigh of relief before sending a mildly disappointed glare over to Karl.
