For example, a non-magical axe can be enchanted three times to get a +3 bonus to attack and damage rolls, while a Berserker Axe already has a +1 to attack and damage rolls, so can it only be enchanted two more times to reach +3 or can it be enchanted three times for a total of +4?
What system for enchanting weapons are you referring to? I don't see anything about limits on re-enchanting weapons in the DMG section on crafting magic items, nor in the XGtE section on the same.
I'm basing this on the fact that in the DMG no weapon or armor in the magic item section has a bonus over +3. And also no bonus that comes from other sources can go over +3. I know bonuses can stack, but I'm not sure if an individual bonus can go over +3.
Ah I see. 5E isn't like prior editions, where magical weapon abilities had a +X equivalent value, and items were tiered to be power-level appropriate. In 5E, a +1 sword may be an appropriate item all the way up through level 20, see the table in the DMG Chapter 1 for appropriate equipment at higher tiers of play.. You're welcome to hand out stronger magic items, or weaker ones, or no ones, or many ones, because having magic items isn't really assumed as part of the power curve in 5E tiers of play.
I don't know of any core rules that cover 'enchanting' magic weapons - the rules cover creating magic weapons (but not further manipulation)....whether the raw materials were an already existing magic weapon or lumps of iron doesn't matter (other than you could argue the already existing magic weapon represents a large portion of the required raw materials - so you're then turning a +2 sword into a +3 sword) - so when it comes to enchanting, you're already in homebrew territory. and since you're there, its homebrew, so there's really no +3 limit (although don't know many DM's who'd allow a +4).

The highest you can get is an Artrifact/Legendary magic item.
Per Raw the highest bonus to hit and damage is +3, but you can add other enchantments such as Berserker on top of the +3.
An example of this is the +3 Axe of the Dwarvenlords that also increases Con by +2 and acts as a sword of sharpness among other things.
