I had a fighter that died because of friendly fire over an AoE spell. I was piiiiisssed. I didn't complain or anything, but now that I think about it, I still should have called that player out to make sure that never happens again. Hurting other party members for the sake of "it's what my character would do" and/or still following through with it after a warning from the DM is beyond cringe. I was still new to D&D at the time, so didn't know how to handle those problematic situations when all I wanted to do was play.

My human gloomstalker was turned to stone by a medusa. Were all super low-level so we have no way of bringing her back. On the plus side I get to try one of the UA races now.

One more for her sculpture garden. Who knows maybe you can go back and restore them at some point. Or DM might have them become the next villain some time down the road "WTF You guys just left me there?"
Lost Mine of Phandelver. My first D&D experience (as a player - I'd DMed a coupla times). The four goblins that you encounter at the very start of the campaign produced a TPK.
My poor wizard took one hit and dropped. worst combat encounter luck I've ever run across. Particularly considering that when I DMed it a year later it turns out it recommends the goblins just knocking the party unconscious in this scenario.
That's rough! My party had trouble with the bugbear boss you find in the cave not long after the road encounter. They were saved from a TPK by a very lucky player critical.
Rook was my child a satyr ranger, level 1. He was one of the guardians of a unicorn's grove hidden in the mountains, and fled when it was destroyed, for fear that the survivors might blame him for outsiders getting into the camp. When he showed up in the campaign, he rolled 3 nat 1s and promptly died without the rest of the party ever knowing his name.
Generally, when a character dies so quickly in a game, I just let them start over (if I'm the DM). It makes very little difference and saves them from having to create another character or re-introduce their character.
Luck that they survived?
luck that you lost 3 characters in one combat.
Just a very tough boss fight. We didn't even manage to kill the boss in the end. We completed the objective, but ended up fleeing rather than winning.

What were you fighting? I have done the same thing when the fighter in the party was completely/partially surrounded.
