I play in a recurring campaign where instead of death saves, we have a "resurrection mechanic" that has both cool aesthetic and character consequences so that any time we die we almost immediately come back, but with mechanical changes to our characters. In Season 1, part of my ranger's res involved her fox companion retrieving her soul to return to her body, but being only a small critter he brought back a little less every time (allowing for the consequence where she was slowly being possessed by her ******* chaotic ex whom she had killed pre-campaign). At a certain point I had to miss a week of play, so we decided to not have my character not res when she normally should have. When she did come back, her return was an incredibly brutal reentry where she had spent six months in a horrifying purgatory (as opposed to the rest of the party's two days), had completely forgotten who one of her two other party members was (because of his res mechanic), and was carrying a blood-stained chunk of fur from the fox, but not enough for anyone to know what really happened to him.
In Season 2 I played her many-times-great-granddaughter, a cleric whose Shield of Faith appeared as a spectral fox (and Spiritual Guardians were the S1 char and her ex), and we all hope that maybe someday we'll revisit the OG party and find out what actually happened to the whole group...but mostly what happened to the fox.

Druid did a flamestrike (3rd Ed) spell. Wizard's Familiar got in the way. Toasted familiar. Druid tried to bring it back with reincarnation - bad result -> it came back as a nymph. Nymph had such a high charisma that its dazzling beauty nearly killed the wizard due to a failed save. Had to wait for the cleric and the others to polymorth it back into a familiar.
