1. 12 AC is painfully low for a CR 12 monster. Maybe add some natural armor? 2. I would suggest having at least 13 (3d8) hit points. If you want to keep it at 1d8, keep in mind that the average is 4, not 1. 3. Change the speed to at least 30 feet hover. 4. You don't need to give it stunned or incapacitated immunity. Consider resistance to nonmagical attacks, though. 5. Most undead speak 'the languages it knew in life'. 6. There's no official meaning of 'outside of combat.' I would instead take that part out and replace it with immunity to nonmagical attacks and resistance to all other damage besides psychic, force, and radiant. 7. Slaying Visage is too powerful to be an unlimited use action. Maybe change it to (1/Day), since creatures that succeed their save against Horrifying Visage can't be affected by it again anyway. I would suggest adding a weaker ability with unlimited uses that works on any creature. 8. I would re-word this to, "While the mirror dead is invisible, she can teleport to a reflective surface that she can see within 60 feet of her."
Overall, I love the concept of this monster, but it might need a few tweaks.

The low AC and HP are because you cannot attack it.
I'm not sure your players will like that... even the eidolon (the monster most similar to yours) is able to be attacked.

That might be because you cannot publish races with words not recognized by whatever puts the red line under words, including the many in D&D.
You can't have any dashes or stuff. But you should be fine with words that are made up.

Thank you. All my previous attempts to create a race have failed miserably and this is the only one I have thought about publishing, so I don't know much about that.
I kinda like the Mirror Dead, but it seems like more of a plot point or quest hook than an enemy.
