This item was based on The Flower of Fates from Adrian Selby's fantastic novel, Snakewood. If you want to put something into your player's hands that gives them a once-only, do-and-die moment, then this is the item for you.
I am about to introduce this item to my campaign, into the hands of level 9 characters. Will it ever get used? Maybe, maybe not. But I want my players to know that if everything goes to hell, they have the opportunity to make one last glorious sacrifice. If a TPK looks inevitable, then maybe one of them will use their Flower of Fates and go out in a blaze of glory.
This shimmering golden potion contains the intact flower known as The Flower of Fates, both glorious and deadly.
A willing creature may consume the potion as a Bonus Action. On taking the potion, you return to your maximum hit points. For the next minute (including the turn on which you take the potion), you may make two actions on each of your turns, you have advantage on attack rolls and ability checks, creatures have disadvantage on saving throws against spells that you cast, and you have resistance to all forms of damage. At the end of the sixth turn after you consume this potion, you die. The power of the poison is such that the affected creature cannot be resurrected by any means short of a Wish spell.
That is such a cool idea! If such an item is ever used it would be a moment that would never be forgotten! I can see someone using it on a final boss fight, especially if it is otherwise very hard.
