What's the most outlandish and hilariously improbable thing that's ever happened in a game you've been a part of?
We were infiltrating a fortress. The mages cast invisibility 10 ft and, while we were making our way past the mines behind the fortress one of the mages, who has always been a bit of a loose cannon, decides he's going to cast sleep on some guards we were passing. He failed, became visible, and began to freak out. The other mage, hoping to scare off the guards casts an illusion spell making it appear like the loose cannon is turning into a werewolf. My ranger has an extreme hatred towards werewolves and doesn't realize it's an illusion. In fact, he fumbles the save and believes wholeheartedly in the farce. So naturally, he attacks this ally turned werewolf with his +5 vorpal longsword. Ko's the mage instantly, but the illusion of the werewolf is still there and now the guards are running off to alert the rest of the stronghold. The remaining mage decides he's going to lead my ranger away from the loose cannon, so he makes the image of the werewolf's face split open revealing a nightmarish, fiery skull, sprout dragon's wings, and fly away. Of course, I get a check to see if I believe such an insane show of magic. There goes the second fumble of the night. He chases the damn thing down. Now the party's dealing with fighting off a hoard of orcs that came pouring out of the fortress. A quarter of the party ends up trapped in the mine with the unconscious loose canon and eventually myself. The rest of the group gets captured and thrown in the fortress's meager prison. This was one of the wildest game nights we had in a long time and I had to very heavily abridge it, but there you go.

PC fails death saving throws = PC now owes hermit replacement material component for revivify. The small tribal village whose warriors killed him might have such a thing. Keeps him alive; gives him a contact; gives me a chance to introduce the player to the death/resurrection system in D&D; provides a roleplaying opportunity, "This body hurts. . ."; gives me a few minor adventure hooks; I don't remember what else.
If you were a sorcerer, where would you have gotten your magic?

Back in the good ol' days (read: 3.x) Mechanus had Inevitables, which ranged from lawful stupid robots to robo-centaurs to Doctor Who-esque steampunk bots to big black metal guys who beat the crap out of vampires to cubes which kill wannabe gods. All inevitables were powerful (above CR 9) while now all modrons are practically the same crappy robot which remains under CR 3.

Pretty terrible. It didn't even happen, as the team found a cannon to blast the sky, breaking the underdark somehow.
