The infected creature suffers one level of exhaustion, and it regains only half the normal number of hit points from spending Hit Dice and no hit points from finishing a long rest. An effect that removes exhaustion reduces its level as specified in the effect's description, with all exhaustion effects ending if a creature's exhaustion level is reduced below 1.
Finishing a long rest reduces a creature's exhaustion level by 1, provided that the creature has also ingested some food and drink. Also, being raised from the dead reduces a creature’s exhaustion level by 1.
Strictly following the rules, I would assume that an infected character that fails their saving throw would increase their exhaustion level by 1 and then they would immediatelly reduce it by 1, because they've just finished their long rest. In other words, in worst case the player's exhaustion level will remain unchanged, which sounds kinda lame (we're dealing with plague here and that should be no joke).
Unless the first paragraph from Exhaustion actually means that in case the disease mentions anything about exhaustion, you shouldn't apply the mechanics described in the second paragraph (reducing exhaustion level at the end of thelong rest). This would make the disease a lot more dangerous, however I'm not sure if it's in line with the rules.

I think you are drastically overstating the severity of the disease, nothing in its description gives me the impression that it should be a major/serious problem.

The first paragraph talks about effects that reduce the condition, nothing else. It seems that you are still trying to up the severity of the disease for some reason. There are diseases that are more severe that has text about not being able to remove Exhaustion until the disease is cured, Sewer Plague doesn't so it would follow the normal rules for getting rid of Exhaustion.
There are certain effects which prevent you removing a level of exhaustion at the end of a long rest.
Since this is not mentioned in the sewer plague wording you quoted, then indeed the character will recover a level of exhaustion and then make the saving throw to avoid losing it again. Just think of it as making a DC 11 saving throw to be allowed to use the general rule of recovering a level of exhaustion at the end of a long rest.
